Abstract: | Die Verengung der Halsschlagader, die Carotisstenose, stellt einen nicht unerheblichen
Risikofaktor für Schlaganfälle dar. Bei ca. 170.000 Schlaganfällen
pro Jahr in Deutschland, geht man von ca. 30.000 Carotisstenosen-assoziierten
Schlaganfällen aus. Dem vorzubeugen gibt es eine interventionelle
und chirurgische Therapiemöglichkeit. Die chirurgische Methode der Endarteriektomie
ist seit langer Zeit etabliert, wohingegen das interventionelle
Carotisstenting noch relativ neu ist und ständig weiterentwickelt wird. Zur
Diskussion stehen momentan Protektionssysteme, die verhindern sollen,
dass während des Stentings zu viele Embolieparitkel nach distal gelangen
und Komplikationen verursachen. Ob die Protektion tatsächlich einen Vorteil
bringt, wurde in der Provenon-Studie untersucht, da in verschiedenen Studien
bisher keine einheitlichen Ergebnisse gezeigt werden konnten. Es gab
zwei Randomisierungsarme, der eine wurde mit, der andere ohne Protektion
gestentet. Verwendet wurden proximale Protektionssysteme, wobei mittels
Okklusion, Flussumkehr und einem externen Filter die Partikel abgefangen
werden sollten. Mittels MRT wurden die DWI-Läsionen, die die zerebralen
Ischämien anzeigen, vor und nach der Prozedur aufgezeichnet. Aufgrund der
Beendigung der Produktion der Protektionssysteme durch den Hersteller
wurde die Studie nach 56 Patienten abgebrochen. Bei dieser niedrigen Patientenzahl
konnte kein statistisch signifikanter Vorteil der Protektionssysteme
gezeigt werden. Auch beim sekundären Studienziel – Einfluss der MRTMorphologie
der Plaques auf die Rate der DWI-Läsionen, Schlaganfall und
Tod – konnte keine eindeutige Korrelation gezeigt werden. Insgesamt wäre
eine höhere Patientenzahl wünschenswert gewesen. Es werden noch mehr
randomisierte Studien zur Protektion erforderlich sein, um genauer zu klären,
wo das Potential dieses Systems liegt und wie man es verbessern könnte. With approximately 170,000 strokes per year in Germany, approximately 30,000 carotid stenosis-associated strokes are supposed. To prevent this, there is an interventional and surgical therapy option. The surgical method of endarterectomy has been established for a long time, whereas the interventional carotid stenting is still relatively new and is constantly evolving. Cerebral protection devices are currently under discussion to prevent that during stenting too many embolic particles access distally and cause complications. If the protection actually brings an advantage was investigated in the Provenon trial, because only inconsistently results were shown in various studies so far. There were two randomization groups, one was stented with and one without protection. Proximal protection devices were used, which means that the particles were trapped by occlusion, flow reversal and an external filter. The DWI-lesions that indicate the cerebral ischemia were recorded by MRI before and after the procedure. Due to the termination of the production of the protection devices by the manufacturer, the study was discontinued after 56 patients. At this low number of patients, no statistically significant benefit of the protection devices could be demonstrated. Also in the secondary study objective - influence the MRI morphology of plaques on the rate of DWI lesions, stroke and death - no clear correlation could be shown. Overall, a higher number of patients would have been desirable. There are even more randomized trials to protection required to clarify where the potential of this system is and how to improve it. |
