Abstract: Hintergrund Vermeidbare Fehler gefährden die Patientensicherheit in der Medizin und sind häufig auf den „human factor“ zurückführen. Positiv beeinflussen lassen sie sich durch eine Verbesserung der Kommunikation und Teamarbeit. Aktuell existiert wenig Forschung zum „human factor“ in der Notfallmedizin. Vor diesem Hintergrund wurde das 7‑4-7-Briefing entwickelt, in dem sieben Themen in sieben Minuten besprochen werden. Dies soll die Teamzusammenarbeit und Kommunikation verbessern, um vermeidbare Fehler zu reduzieren. Material und Methoden Das 7‑4-7-Briefing wurde in Deutschland im bodengebundenen sowie luftgebundenen Rettungsdienst über zwei Monate etabliert und evaluiert. Dabei wurde mittels Prä- und Postfragebogen untersucht, ob das Briefing einen Einfluss auf die Arbeitszufriedenheit, den Rollenkonflikt, die Rollenklarheit, das Gemeinschaftsgefühl und das empfundene Stresserleben hat. Anschließend wurde das Briefing von den Probanden subjektiv evaluiert. Ergebnisse Insgesamt zeigte sich keine Verbesserung der Arbeitszufriedenheit, der Rollenkonflikte, der Rollenklarheit, des Gemeinschaftsgefühls und des Stresserlebens. Für die Subgruppe der Ärzte konnte eine Verbesserung des Stresses im Faktor schwer einzuschätzende Einsätze nachgewiesen werden (p < 0,05). Besonders hervorzuheben ist, dass das Briefing von den Probanden signifikant als positiv bewertet wurde und sowohl taktische als auch medizinische Inhalte des Briefings als relevant eingeschätzt wurden (p < 0,05). Diskussion Eine positive Evaluation des 7‑4-7-Briefings ermöglicht eine dauerhafte Integration des Briefings in den Arbeitsalltag. Da ein Teilnutzen des Briefings aktuell nur für die Subgruppe der Ärzte quantifiziert werden konnte, sind hier prospektiv weitere Studien notwendig.
Background Patient safety is threatened by errors in medicine. The human factor is a reason for avoidable errors. These avoidable errors can be positively influenced by improving communication and teamwork [21, 34]. Human factors are still not researched enough in emergency medicine. With that said the 7‑4‑7 briefing for emergency medicine workers was developed in which seven topics are discussed within seven minutes. This briefing aims to strengthen teamwork in order to reduce avoidable errors in patient care. Materials and methods This study implemented the 7‑4‑7 briefing for 2 months in Germany for in-flight and ground-based emergency medicine teams. The briefing was evaluated with a pre- and postquestionnaire. The aim of the study was to show whether the briefing had an impact on work satisfaction, role conflict, role clarity, teamwork, and perceived stress. Following this, the participants evaluated the briefing itself. Results There was no significant improvement on work satisfaction, role conflict, role clarity, teamwork, or perceived stress. However, there was a significant reduction of perceived stress for the group of emergency physicians for emergencies which were difficult to assess (p < 0.05). Notably, the briefing was significantly positively evaluated over all participants and all, medical and technical, contents were evaluated as relevant to the briefing (p < 0.05). Conclusions This study shows that the 7‑4‑7 briefing is an accepted and well evaluated tool, which can be easily implemented into everyday work. Further research must aim to quantify the briefings value beyond the population of emergency physicians and the factor stress.
