Title: Mechanische Thrombektomie bei distalen Verschlüssen der Arteria cerebri media: Technische Durchführbarkeit, Sicherheit und klinisches Outcome

Abstract: Die mechanische Thrombektomie ist als Standardtherapie bei proximalen Gefäßverschlüssen der vorderen Strombahn etabliert und wird leitlinienbasiert auch für M2-Verschlüsse empfohlen, wenngleich diese Empfehlung überwiegend auf Subgruppenanalysen proximaler M2 Verschlüsse beruht. Unklar bleibt zudem, ob der in DAWN und DEFUSE 3 belegte Nutzen im erweiterten Zeitfenster bis 24 Stunden auf distale M2- und M3-Verschlüsse übertragbar ist. Die vorliegende Arbeit untersucht daher die technische Durchführbarkeit, Sicherheit und klinischen Ergebnisse der Thrombektomie bei distalen M2- und M3-Verschlüssen im Vergleich zu proximalen Verschlusslokalisationen und analysiert zusätzlich die Behandlungsergebnisse im frühen und erweiterten Zeitfenster. Zudem werden Prädiktoren eines günstigen funktionellen Outcomes identifiziert. Untersucht wurde eine retrospektive monozentrische Kohorte von 218 Patienten, die zwischen Januar 2020 und August 2023 mittels mechanischer Thrombektomie behandelt wurden (123 M1-, 60 proximale M2- und 35 distale M2+M3-Verschlüsse). Zusätzlich erfolgte eine Analyse der M2+M3-Verschlüsse im frühen (≤ 6 h) und späten (> 6 h) Zeitfenster. Ausgewertet wurden klinische Daten, bildgebende Befunde, zeitliche Intervalle, technische Parameter sowie Komplikationen. Die statistische Analyse umfasste univariate Tests und multivariate logistische Regressionsmodelle. Über alle Verschlusslokalisationen hinweg zeigte sich eine signifikante Verbesserung der National Institutes of Health Stroke Scale-Werte bis zur Entlassung. Ein günstiges funktionelles Outcome (modifizierte Rankin-Skala ≤ 2) wurde bei 28,1 % der Patienten mit M1- , bei 40,8 % mit proximalen M2- und bei 50,0 % mit distalen M2+M3-Verschlüssen erreicht. Auch im erweiterten Zeitfenster zeigten Patienten eine klinische Besserung, wenngleich mehr Thrombektomiemanöver erforderlich waren und First-pass-Rekanalisationen seltener waren. Die Komplikationsraten, einschließlich symptomatischer intrakranieller Blutungen und prozeduraler Ereignisse (Perforation, Dissektion), waren in allen Gruppen niedrig und unterschieden sich nicht signifikant untereinander. In der multivariaten Analyse erwiesen sich höheres Lebensalter, höhere initiale National Institutes of Health Stroke Scale-Werte und eine größere Anzahl erforderlicher Manöver als unabhängige negative Prädiktoren eines günstigen Outcomes. Die Ergebnisse dieser Kohorte deuten darauf hin, dass die mechanische Thrombektomie bei distalen Verschlüssen sicher durchgeführt werden kann und auch im erweiterten Zeitfenster 2 klinische Verbesserungen ermöglicht. Gleichzeitig ist zu berücksichtigen, dass distale Verschlüsse häufig mit milderen klinischen Defiziten einhergehen und potenzielle prozedurale Risiken daher sorgfältig abgewogen werden müssen. Aufgrund der begrenzten Fallzahl und des retrospektiven Studiendesigns sollten die Befunde zurückhaltend interpretiert und in zukünftigen prospektiven Studien weiter validiert werden.
Mechanical thrombectomy is firmly established as the standard therapy for proximal large- vessel occlusions of the anterior circulation and is also recommended in current guidelines for occlusions of the M2 segment. However, this recommendation is largely based on subgroup analyses of major randomized controlled trials, in which predominantly proximal M2 occlusions were included. Moreover, it remains uncertain whether the benefit of thrombectomy in the extended time window of up to 24 hours after symptom onset, as demonstrated in the DAWN and DEFUSE 3 trials, can be extrapolated to distal M2 and M3 occlusions. This study therefore evaluates the technical feasibility, safety, and clinical outcomes of thrombectomy for distal M2 and M3 occlusions in comparison with established proximal occlusion sites and additionally analyzes treatment results in both the early and extended time windows. Predictors of favorable functional outcome are also identified. A retrospective single-center cohort of 218 patients treated with mechanical thrombectomy between January 2020 and August 2023 was analyzed (123 M1, 60 proximal M2, and 35 distal M2+M3 occlusions). A separate analysis was performed for M2+M3 occlusions treated in the early (≤ 6 h) and late (> 6 h) time windows. Clinical data, imaging findings, time intervals, technical parameters, and complications were evaluated. Statistical analyses included univariate tests and multivariate logistic regression models. Across all occlusion sites, National Institutes of Health Stroke Scale scores improved significantly from admission to discharge. A favorable functional outcome (modified Rankin Scale ≤ 2) was achieved in 28.1% of patients with M1, 40.8% with proximal M2, and 50.0% with distal M2+M3 occlusions. Patients treated in the extended time window also showed clinical improvement, although more thrombectomy maneuvers were required and first-pass reperfusion was achieved less frequently. Complication rates, including symptomatic intracranial hemorrhage and procedural events such as perforation or dissection, were low and did not differ significantly between the groups. In multivariate analysis, higher age, higher 3 baseline National Institutes of Health Stroke Scale scores, and a greater number of required thrombectomy maneuvers emerged as independent negative predictors of a favorable outcome. The findings of this cohort indicate that mechanical thrombectomy can be performed safely in distal occlusions and may lead to clinical improvement even in the extended time window. Nevertheless, distal occlusions often present with milder neurological deficits, and potential procedural risks may therefore weigh more heavily in the risk–benefit assessment. In light of the limited sample size and the retrospective study design, these findings should be interpreted with caution and validated in future prospective studies.
