Abstract: Hintergrund: Die akute Mastoiditis (AM) stellt eine sehr seltene, aber schwere Komplikation der akuten Otitis media (AOM) dar. Infektionen der Atemwege sind die Hauptindikation für eine ambulante Antibiotikatherapie (ABT) im Kindes- und Jugendalter. Das Saarland zählt zu den Bundesländern mit der höchsten Antibiotikaverordnungsrate (AVR) in Deutschland. In den letzten 10 Jahren ist die AVR im Saarland v.a. bei Säuglingen und Kleinkindern deutlich zurückgegangen. Ziele und Fragestellung: Ziel des retrospektiven Mastoiditis-Audits im PaedineSaar Netzwerk ist es, die Inzidenz der AM im Saarland (2014-2019) im Hinblick auf die sinkende AVR im ambulanten Bereich bei Kindern zu untersuchen sowie anonymisierte klinische Verlaufsdaten zur AM zusammenzustellen. Zusätzlich wurde die Umsetzung der 2013 eingeführten klinikinternen Leitlinie „Akute Mastoiditis“ der Kinderklinik am Universitätsklinikum des Saarlandes überprüft. Methoden: Es wurden alle stationären AM-Fälle (2014-2019) aus 6 Kliniken für Kinderheilkunde und/oder Hals-Nasen-Ohren-Heilkunde im Saarland und Trier retrospektiv ausgewertet. Patienten1 mit akuter Mastoiditis im Alter von 0 bis einschließlich 17 Jahren mit Wohnsitz im Saarland wurden in die Studie eingeschlossen und anhand ICD-10, DRG sowie Mastoidektomie-Kodierungen identifiziert. Ergebnisse: Zwischen 2014 und 2019 wurden 53 stationär behandelte AM-Fälle erfasst. Davon waren 60,4% männlichen Geschlechts; das mediane Alter betrug 3 Jahre. Die mittlere Inzidenz der akuten Mastoiditis lag bei 6,1/100.000 Kindern und Jugendlichen (Minimum 2,8/100.000 2016, Maximum 8,2/100.000 2018). Während des Studienzeitraums gab es keinen signifikanten Anstieg der Inzidenz. Die Mehrheit der Patienten zeigte die typische Symptomatik der AM mit Otalgie, retroaurikulärer Schwellung/Rötung und Fieber. Bei 66% der Patienten wurde eine Bildgebung durchgeführt (Magnetresonanztomographie, Computertomographie, Röntgen, Sonographie). Eine Erregerdiagnostik erfolgte in 60,9% der Fälle. In 13,2% der Fälle gelang ein Erregernachweis aus invasiv gewonnenen Proben. Am häufigsten (jeweils zweimal) wurden Streptococcus pneumoniae und alpha-hämolysierende Streptokokken nachgewiesen. 34% der Patienten erhielten schon prästationär eine orale antibiotische Therapie. Mindestens eine Komplikation (Weichteilabszess, Fazialisparese, Sinusvenenthrombose (SVT)) trat bei 30,2% der Patienten auf. Es zeigte sich ein nicht signifikanter Trend zu mehr Komplikationen bei Kindern ohne prästationäre orale Antibiotikabehandlung (14/35 (40%) vs. 2/18 (11%) P=0,056). Zwei Drittel der akuten Mastoiditis Fälle wurde rein konservativ mit einer intravenösen ABT behandelt. Am häufigsten wurden Ampicillin/Sulbactam, Cefuroxim und Cephalosporine der Gruppe III verwendet. 34% der Patienten wurden zusätzlich operativ behandelt (Myringotomie mit/ohne Einlage eines Paukenröhrchens, Mastoidektomie). Fazit: Die Inzidenz der stationär behandelten AM bei Kindern und Jugendlichen mit Wohnsitz im Saarland ist 2014-2019 trotz eines signifikanten Rückgangs der ambulanten AVR nicht signifikant angestiegen. Ein Großteil der AM-Fälle wurde konservativ mit intravenösen Antibiotika (AB) behandelt. Die Ergebnisse dieses Audits sollen zur Entwicklung eines standardisierten Vorgehens bei der Diagnostik und Behandlung von Kindern mit AM dienen.
Background: Acute mastoiditis (AM) is a rare, but serious complication of acute otitis media (AOM). Infections of the respiratory tract are the main indication for outpatient antibiotic therapy in children and adolescents. Saarland is one of the regions in Germany with the highest antibiotic prescription rate (APR). In recent years the APR in the pediatric population has decreased significantly. Objectives: The aim of the retrospective mastoiditis audit in the PaedineSaar network is to investigate the incidence of inpatient AM in Saarland (2014-2019) regarding to the decreasing APRs in children, as well as to gather data of the clinical course of AM. In this context the implementation of AM guidelines at the Saarland University hospital was reviewed. Methods: All inpatient AM cases between 2014 and 2019 were analyzed retrospectively from 6 hospitals for pediatrics and/or otorhinolaryngology in Saarland and Trier. Children and adolescents aged 0-17 years and residing in Saarland were included in the study. The AM cases were identified using ICD-10, DRG and mastoidectomy codes. Results: Between 2014 and 2019 53 inpatient treated AM cases have been recorded. Of these, 60.4% were male; the median age was 3 years. The mean incidence of AM was 6.1/100,000 children and adolescents (minimum 2.8/100,000 2016, maximum 8.2/100,000 2018). There was no significant increase in AM incidence during the study period. Most patients showed the typical symptoms of AM with otalgia, retroauricular swelling/redness and fever. Imaging was performed in 66% of patients (magnetic resonance imaging, computed tomography, X-ray, sonography). A pathogen diagnosis was carried out in 60.9% of the cases. In 13.2% a pathogen was detected intraoperatively or invasively. Streptococcus pneumoniae and alpha-hemolytic streptococci were detected most frequently (twice each). 34% (18/53) of the patients received prehospital antibiotic treatment (main indication: AOM 83%). At least one complication (soft tissue abscess, facial paralysis, sinus vein thrombosis (SVT)) occurred in 30.2% of patients. There was a slight trend towards more complications in children without oral antibiotic treatment before admission (14/35 (40%) vs. 2/18 (11%) P=0.056). Two-thirds of AM cases were treated conservatively with intravenous antibiotic therapy. Ampicillin/sulbactam, cefuroxime and 3rd generation cephalosporins were used most frequently. 34% of the patients underwent additional surgical treatment (myringotomy with/without insertion of a ventilation tube, mastoidectomy). Conclusions: The incidence of AM leading to inpatient treatment in children in Saarland did not increase 2014-2019 despite a significant and sustained decline in the outpatient APRs. Most AM cases were treated conservatively with intravenous antibiotics. The results of this audit should be used for the development of a more standardized approach concerning the diagnostics and treatment of children with AM.
