Abstract: Hintergrund Patienten mit bikuspider Aortenklappe entwickeln im Laufe ihres Lebens häufig eine Aortenklappenstenose oder Insuffizienz. Für beide Arten von Pathologien gilt der Aortenklappenersatz heutzutage als Standardtherapie, obwohl Patienten mit Aortenklappenin-suffizienz von einer Rekonstruktion ihrer Klappe anstelle eines Ersatzes merklich profi-tieren. Aufgrund der hohen chirurgischen Komplexität einer Aortenklappenrekonstruktion kann dieses OP-Verfahren jedoch nicht jedem Patienten mit bikuspider Aortenklappe angeboten werden. Daher wird in dieser Arbeit die rekonstruktionsrelevante Geometrie der bikuspiden Aortenklappe untersucht und in einem Klassifikationssystem präsentiert. Zudem werden echokardiographische Messverfahren geprüft, die die Anwendung des vorgestellten Klassifikationssystems ermöglichen. Methoden Im Zeitraum von September 2015 bis September 2017 wurde die Geometrie der Aortenklappe von 106 Patienten mit bikuspid angelegter Klappe erfasst. Alle Patienten wurden anhand der kommissuralen Orientierung in drei Gruppen eingeteilt (Typ C/120°-140°, Typ B/140°-159°, Typ A/160°-180°). Zusätzliche wurde präoperativ mithilfe der Echokardiographie die Anatomie, insbesondere die Dimensionen der Aortenwurzel, erfasst. An-schließend erfolgte eine intraoperative Vermessung der Geometrie des Taschenapparates. Alle Messungen wurden in den Zusammenhang mit der kommissuralen Orientie-rung bzw. dem Klassifikationssystem gebracht. Anschließend wurde an einer Subpopulation von 62 Patienten die Reliabilität verschiedener Messverfahren zur Bestimmung der kommissuralen Orientierung untersucht. Ins-besondere wurde eine einfache Messung über die Koaptationslinie mit geometrischen Konstruktionen des Wurzelmittelpunktes verglichen. Ergebnisse Die kommissurale Orientierung beträgt median 150 Grad (IQA:140-160) und kann Werte im Bereich von 120 bis 180 Grad annehmen. 40 (37,7%) aller Patienten hatten ein Aneurysma der Aorta. Die Häufigkeit (p=0,311) und Phänotypisierung (p=0,678) von Aneu-rysmen zeigten keine signifikante Assoziation zum Klassifikationssystem. Die geometrische Höhe der nicht fusionierten Tasche war bei Klappen des Typs C (21 mm) niedriger (p=0,001) als bei Taschen des Typs B (23 mm) und des Typs A (23 mm). Die Höhe der fusionierten Kommissur hat eine lineare Beziehung (y= -0,191x Zusammenfassungen Lennart Froede 2 + 43,657, R2=0,332) zur kommissuralen Orientierung (p<0,001). Patienten mit Typ C (19,3 mm ± 3,8 mm) Klappe hatten eine höhere (p<0,001) fusionierte Kommissur als Patienten mit Typ B (12,6 mm ± 4,3 mm) oder Typ A (5,3 mm ± 3,5 mm) Klappe. Die Fusionslänge steht ebenfalls in linearem Zusammenhang (y=0,264x - 26,322, R2=0,599) mit der kommissuralen Orientierung (p<0,001). Patienten mit Typ A (16,5 mm ± 4,1 mm) Klappe hatten eine ausgeprägtere Fusion (p<0,001) als solche mit Typ B (13,0 mm ± 3,1 mm) oder Typ C (7,4 mm ± 2,5 mm) Klappe. Messverfahren für die kommissurale Orientierung, die auf geometrischen Mittelpunkt-konstruktionen (149,2° ± 12,5°) basieren, haben eine hohe Reliabilität (ICC>0,98). Die Messung in Systole oder Diastole liefert keine unterschiedlichen Ergebnisse (ICC=0,993). Die Messung des Winkels über die Koaptationslinie (138,2° ± 14,4°) zeigt eine geringere Reliabilität (ICC=0,783) und misst im Durchschnitt einen 11° ± 8,9° klei-neren Winkel (p<0,001). Schlussfolgerung Trotz vieler bisher bekannter Klassifikationssysteme für bikuspide Aortenklappen liefert die quantitative Beurteilung der Geometrie bisher unbekannte Erkenntnisse. Die kom-missurale Orientierung der bikuspiden Aortenklappe folgt einem kontinuierlichen Spektrum (120°-180°). Sie steht in direktem Zusammenhang mit der Fusionslänge und der Höhe der fusionierten Kommissur. Die Einteilung der Aortenklappe in drei Gruppen zeigt trotz Überschneidungen eine hohe Assoziation zu verschiedenen Techniken der rekonstruktiven Aortenklappenchirurgie. Ein Prolaps bei sehr asymmetrischen Klappen des Typs C wird häufig vergleichbar zu trikuspiden Aortenklappen korrigiert. Bei asymmetrischen Klappen des Typs B wird ein Prolaps meistens durch einen Verschluss der Fusionslinie behoben. Bei symmetrischen Klappen des Typs A erfolgt in der Regel eine zentrale Plikatur der prolabierten Tasche. Zur klinischen Anwendung wird ein reliables Messverfahren der kommissuralen Orientierung benötigt. Hierfür bietet sich die Konstruktion des Mittelpunktes mithilfe einer halbierten Strecke zwischen fusioniertem und nicht fusioniertem Sinus an. Diese Technik ist mit jedem Echokardiographiegerät einfach praktizierbar und führt zu verlässlichen Ergebnissen.
Background The most common pathologies associated with bicuspid aortic valves are aortic stenosis and aortic regurgitation. Both are treated regularly with aortic valve replacement, alt-hough it is well known that aortic valve repair leads to a better outcome for patients suffering from aortic regurgitation. Only a limited number of surgeons are able to offer their patients aortic valve repair because it is a particularly complex surgical procedure. Therefore, the following paper has the primary intention to investigate the relevant ge-ometry of the bicuspid aortic valve and introduce a repair orientated classification. Addi-tionally, new echocardiographic measurement methods are tested to further simplify the clinical application of the classification. Methods During September 2015 until September 2017 the geometry of 106 bicuspid aortic valves has been investigated. All patients were divided into three groups (type C/120°-140°, type B/140°-159°, type A/160°-180°) according to the commissural orientation of their valves. Preoperatively, all patients have undergone echocardiography and their valves, especially, the aortic dimensions have been assessed. Afterwards, the valvular geometry was measured intraoperatively. All results were analyzed in the context of the classifica-tion and the commissural orientation. In a subpopulation, containing 62 patients, new methods measuring the commissural orientation of bicuspid aortic valves have been explored. Different techniques using ge-ometric constructions for the determination of the aortic roots center as vertex for angle measuring have been compared to a simple approach using the valvular coaptationline. Results The commissural orientation of bicuspid aortic valves ranged from 120° to 180°, with a median of 150° (IQR 140-160). Forty (37.7%) of all patients had an aortic aneurysm. The prevalence of aneurysms (p=0.311) and phenotypes (p=0.678) did not show a significant association to the classification. The geometric height of the non-fused cusp was lower (p<0.001) in type C (21 mm) compared to type B (23 mm) and type A (23 mm) valves. There is a linear dependence (y= -0.191x + 43.657, R2=0.332) between the height of the fused commissure and the commissural orientation (p<0.001). The fused commissure in patients with type C (19.3 mm ± 3.8 mm) valves is higher (p<0.001) than in patients with type B (12.6 mm ± 4.3 mm) and type A (5.3 mm ± 3.5 mm) valves. There is also a linear dependence (y=0.264x – 26.322, R2=0.599) between the length of fusion and the com-missural orientation (p<0.001). Patients with type A (16.5 mm ± 4.1 mm) valves had a more pronounced fusion length (p<0.001) compared to patients with type B (13.0 mm ± 3.1 mm) and type C (7.4 mm ± 2.5 mm) valves. The methods measuring the commissural orientation based on a geometric approach (149.2° ± 12.5°) are reliable (ICC>0.98), independently, whether they were applied in systole or diastole (ICC=0.993). The coaptationline approach (138.2° ± 14.4°) is less reliable (ICC=0.783) and underestimates (p<0.001) the commissural orientation by 11° ± 8.9°. Conclusion The quantitative study of the anatomy of the bicuspid aortic valve improves the under-standing of valvular geometry significantly, although there are already a lot of classifica-tions existing. The commissural orientation of the bicuspid aortic valve is a continuous variable (120°-180°). The height of the fused commissure and the length of fusion are depending on commissural orientation. Different aortic valve repair strategies are markedly related to the classification, although they are partially overlapping between the groups. In very asymmetric bicuspid aortic valves (type C) prolaps correction is quite similar as it is in tricuspid aortic valves. In asymmetric bicuspid aortic valves (type B) a prolaps will be corrected regularly be direct closing of the fused commissure. In symmetric bicuspid aortic valves (type A) a prolaps will be corrected most times by central plication of the affected cusp. To use the classification in the daily clinical practice effectively, a reliable method meas-uring the commissural orientation of a bicuspid aortic valve is required. The simplest way to determine the commissural orientation of bicuspid aortic valves is the geometric con-struction of the aortic roots center using a line segment. Therefore, a line segment will be drawn from the fused commissure to the middle of the non-fused sinus. Subsequently, the middle of the line segment will be used as vertex to measure the commissural angle. This method could be applied easily with every common echocardiography device and leads to reproducible and reliable results.
