Abstract: | Chronische Wunden, die im Vergleich zu kardiovaskulären oder neurologischen Erkrankungen in unserer Gesellschaft seltener auftreten, bleiben auch in der Literatur bisher wenig erforscht. Und das obwohl das komplexe Krankheitsbild chronischer Wunden auf physischer und psychischer Ebene mit längerfristigen Belastungen einhergeht. Bereits existierende Literatur rückt die Folgen chronischer Wunden auf die Patienten in den Fokus, während die Auswirkungen auf Angehörige weitestgehend unberücksichtigt bleiben. Dabei spielen Angehörige nicht nur für die Patienten eine wichtige Rolle, da sie ihnen oftmals den Schritt ins Pflegeheim ersparen, sondern stellen auch für die Gesellschaft aufgrund von Kostenersparnissen eine wesentliche Säule dar. Gerade im Hinblick auf die alternde Bevölkerung und die Tatsache, dass die chronische Wunde eine Erkrankung des alten Menschen darstellt, lässt der Erforschung dieses Krankheitsbilds und der Betroffenen eine wachsende Bedeutung zukommen. Das Ziel dieser Studie war es, die Belastungen, Lebensqualität und Bedürfnisse pflegender Angehöriger von Patienten mit chronischen Wunden aus veröffentlichter Literatur zu erforschen, um Entlastungsangebote in diesem Bereich voranzutreiben. Dazu wurden im Juni 2020 die medizinische Datenbank „Pubmed“ sowie Literaturverzeichnisse gefundener Quellen systematisch durchsucht und ausgewertet. Dabei zeigte sich, dass pflegende Angehörige von Patienten mit chronischen Wunden vor allem unter Belastungen emotionaler Art leiden und die Lebensqualität von der geleisteten Pflege sowie der Lebensqualität der Patienten abhängt. Als wichtigstes Bedürfnis ergab sich mehr Unterstützung durch Familienangehörige sowie durch professionelle Pflegekräfte, um Unsicherheiten im Umgang mit der Wunde zu reduzieren und die Pflegebelastung auf mehrere Schultern zu verteilen. Es wurde zudem deutlich, dass theoretische Maßnahmen wie das Bereitstellen von Informationsmaterial nicht ausreichend sind und praktische Schulungsmaßnahmen nicht ersetzen. Diese Ergebnisse verdeutlichen, in welchen Bereichen pflegende Angehörige von Patienten mit chronischen Wunden besonders belastet sind und können als Grundlage zur Entwicklung individuell ausgelegter Entlastungsangebote dienen. Eine größere Studienlage wäre notwendig, um die Lebensqualität Angehöriger und deren Beeinflussung in eine positive oder negative Richtung klar beurteilen zu können. Chronic wounds occur less common in our society compared to, for example, cardiovascular or neurological diseases. Little research has been done on it so far, despite the fact that the complex clinical picture is associated with long-term physical and mental burden. Existing literature focuses on the consequences of chronic wounds for the patient, leaving the effects on relatives largely unconsidered. But informal caregivers not only play an important role for the patients preventing them from admission to a nursing home but also represent an essential pillar for society due to cost savings. Especially with regard to the aging population and the fact that chronic wounds are mainly affecting the elderly, research into this disease and those affected by it is becoming increasingly important. The aim of this study was to investigate the burden, the quality of life and needs of informal caregivers of people with chronic wounds from existing literature to further the development of relief offers in this area. A systematic literature review was conducted in June 2020 by searching through the medical database “Pubmed” and reference lists of detected literature. It was found that informal caregivers of patients with chronic wounds suffer primarily from emotional stress and the quality of life depends on the care provided and the quality of life of the patients. The most important need was more support from family members and healthcare professionals in order to reduce uncertainty in dealing with the wound and to divide the burden of care among several people. It was also shown that theoretical measures such as the provision of information material are not sufficient and do not replace practical training measures. |
