Abstract: Endoscopic submucosal dissection (ESD) is rarely considered for pedunculated colorectal lesions due to longer procedure times and higher complication risks, although ESD is highly desirable when direct visual assessment suggests advanced polyp [1,2,3]. Thus, to expedite the procedure, we have introduced a standardized ESD procedure for pedunculated lesions involving the use of a grasping forceps-knife coupled with a large-diameter oblique distal attachment to allow for free intra-cap rotation of the all-in-one ESD device, enabling rapid colorectal ESD for Paris Ip lesions (Fig. 1).
