Title: Einfluss einer Ernährungsberatung auf kurzkettige Fettsäuren und Calprotectin im Stuhl sowie auf erkrankungstypische Symptome bei Patienten mit idiopathischem Parkinson Syndrom

Abstract: Es gibt zunehmend Hinweise darauf, dass die sog. „Gut-Brain Axis“ eine Rolle in der Krankheitsentstehung sowie bei der gastrointestinalen Symptomatik bei Patienten mit idiopathischem Parkinson Syndrom (IPS) spielt. Die genauen pathophysiologischen Zusammenhänge sind Gegenstand aktueller Forschung. Es wird vermutet, dass das Darm-Mikrobiom und von Darmbakterien produzierte Metaboliten, wie beispielsweise kurzkettige Fettsäuren (engl. short chain fatty acids, SCFA), beeinflussende Faktoren dieser Achse darstellen. Den SCFA wird unter anderem eine modulierende Fähigkeit auf die gastrointestinale Motilität zugeschrieben. Ferner wurden eine stabilisierende Wirkung auf die intestinale Barriere und eine anti-inflammatorische Funktion der SCFA festgestellt. Studien zeigen, dass SCFA im Stuhl bei Patienten mit IPS in geringeren Konzentrationen nachweisbar sind. Zudem gibt es Hinweise dafür, dass bei Patienten mit IPS intestinale Entzündungsprozesse eine wichtige Rolle spielen. Messbar ist diese intestinale Inflammation unter anderem an einer erhöhten Calprotectin Konzentration im Stuhl von Patienten mit IPS. In Studien zu anderen Erkrankungen konnte bereits gezeigt werden, dass durch eine angepasste und ausgewogene Ernährung Calprotectin gesenkt und die Konzentrationen einzelner SCFA erhöht werden können. Sollte ein solcher Effekt bei Patienten mit IPS reproduzierbar sein, könnte hieraus unter anderem eine Verbesserung der gastrointestinalen Symptome resultieren. Die vorliegende Arbeit, als Teil der RESISTA-PD Studie, untersucht den Effekt einer Ernährungsberatung auf die SCFA-Konzentration und den Entzündungsmarker Calprotectin im Stuhl sowie auf erkrankungstypische Symptome. Bei RESISTA-PD handelt es sich um eine prospektive Studie, welche sich über einen Zeitraum von acht Wochen erstreckt. Die vorliegende Arbeit beschreibt einen Kontroll-Arm der RESISTA-PD Studie, im Rahmen derer 25 Probanden mit IPS eine Ernährungsberatung erhielten und über den Zeitraum von acht Wochen beobachtet wurden. Vor der Intervention (Baseline) wurden die Probanden klinisch untersucht, erkrankungstypische Symptome wurden unter Zuhilfenahme validierter klinischer Skalen erfasst und die Probanden wurden um Einsendung von Stuhlproben gebeten. Als Intervention erfolgte dann eine Ernährungsberatung basierend auf den Richtlinien der Deutschen Gesellschaft für Ernährung (DGE). Die Probanden gaben zum Zeitpunkt vier Wochen und zum Zeitpunkt acht Wochen weitere Stuhlproben ab, welche auf die Konzentrationen der SCFA und Calprotectin untersucht wurden. Zusätzlich wurden zum Zeitpunkt vier Wochen und zum Zeitpunkt acht Wochen erkrankungstypische Symptome unter Zuhilfenahme standardisierter klinischer Skalen erhoben. Die Ergebnisse zeigten keine statistisch signifikanten Unterschiede in den SCFA Konzentrationen bei Baseline verglichen mit dem gesunden Kontrollkollektiv der RESISTA-PD Studie. Analog zu Beobachtungen anderer Studien, die ebenfalls intestinale Entzündungsprozesse bei Patienten mit IPS beschreiben, zeigten sich jedoch die Calprotectin Konzentrationen der Probanden mit IPS bei Baseline signifikant erhöht (p<0,001). Die durchgeführte Intervention zeigte weder eine signifikante Auswirkung auf erkrankungstypische Symptome, noch auf die Konzentrationen von SCFA oder Calprotectin im Stuhl. Eine Verbesserung der Obstipation nach der Ernährungsberatung gaben 43% der Probanden an. Die Ernährungsberatung setzten 39% der Probanden so um, dass nach acht Wochen ausgewogenere Ernährungsgewohnheiten (entsprechend den Empfehlungen der DGE) vorlagen, welche wir durch einen Score objektivierten konnten. Durch die Intervention traten keine spezifischen Nebenwirkungen auf. Um die maximal erreichbaren Effekte einer Ernährungsberatung (und einer Ernährungsumstellung in der Folge) besser beurteilen zu können, ist für zukünftige Studien vor allem eine engmaschigere und professionelle Begleitung der Probanden während der Ernährungsumstellung sowie ein längerer Beobachtungszeitraum sinnvoll. Möglicherweise scheint weiterhin der Einsatz von Präbiotika ein praktikabler Ansatz, um die erwünschten Veränderungen der SCFA, der intestinalen Inflammation messbar an Calprotectin und eine positive Wirkung auf die multiplen gastrointestinalen Symptome erzielen zu können.
There is rising evidence that the gut-brain-axis plays a significant role in the development of Parkinson‘s Disease (PD) as well as in the development of typical gastrointestinal symptoms in patients with PD. The exact pathophysiological mechanisms are subject of current research. It is being assumed that the gut microbiome and its metabolites, such as short chain fatty acids (SCFA), play an important role in the gut-brain-axis. SCFA have a modulating effect on gastrointestinal motility. They are also known to have a stabilizing effect on the gastrointestinal barrier as well as to exert antiinflammatory functions. Studies analyzing fecal samples of patients with PD show a lower concentration of SCFA. Moreover, there is evidence for intestinal inflammation to play an important role in PD. A potential way to measure this intestinal inflammation is by quantifying the intestinal concentration of calprotectin in feces of patients with PD. Studies concerning other diseases have shown that it is possible to reduce the intestinal concentration of calprotectin and to increase the concentration of SCFA by establishing and maintaining a healthy diet. By reproducing this effect in patients with PD it might be possible to alleviate their typical gastrointestinal symptoms. This thesis is a part of the RESISTA-PD trial. RESISTA-PD investigates the effect of nutritional counseling given to patients with PD and its effects on the concentration of fecal SCFA and calprotectin as well as on typical non-motor symptoms in PD. The RESISTA-PD trial is a prospective trial following subjects over the course of eight weeks. This thesis focuses on a control-arm of the RESISTA-PD trial, following 25 subjects with PD after being counseled for a healthy nutrition. Before the intervention (baseline) all patients were examined clinically, typical symptoms were measured by validated clinical scores and patients were asked to send in fecal samples. As an intervention we gave nutritional counseling based on the guidelines of the German Nutrition Society (DGE). Subjects sent in fecal samples four and eight weeks after baseline. Fecal samples were analyzed for their concentration of SCFA and calprotectin. In addition, we asked for typical symptoms and measured them by applying validated clinical scores four and eight weeks after baseline. The results did not show any statistically significant changes in the concentration of SCFA compared to healthy control subjects from the RESISTA-PD trial, whereas the calprotectin concentration was proven to be significantly elevated (p<0.001) at baseline. This finding is in line with other studies concerning intestinal inflammation in patients with PD. The intervention did neither induce significant changes in typical non-motor symptoms in PD, nor in the concentration of SCFA or calprotectin. Improvement of constipation was reported in 43% of subjects after the intervention. The nutritional advice was implemented by 39% of PD patients, who showed a healthier way of eating after eight weeks, measured by a self-concipated score based on the guidelines of the DGE. Subjects did not report specific adverse effects during the intervention. To be able to analyze possible beneficial effects of nutritional counseling (and as a consequence changes in nutrition) further studies will require frequent and professional nutritional counseling during the intervention as well. Also, performing long-term studies should be taken into consideration. The application of prebiotics might be a feasible approach to induce the anticipated changes in the concentration of SCFA and calprotectin, which again could result in a positive influence on typical gastrointestinal symptoms in PD.
