Abstract: | Bei Patienten im Kindesalter sollte bei auftretender Blasenbildung auch bei geringer
Symptomatik an die Möglichkeit einer hereditären Epidermolysis bullosa gedacht
werden. Diagnostisch wegweisend ist neben Klinik und Histologie die molekulargenetische Untersuchung. Therapeutisch stehen aktuell bei lokalisierten Formen symptomatische topische Therapieoptionen weiterhin im Vordergrund. Besonders interessant ist hier die neue Möglichkeit einer topischen Therapie mit Diacerein 1 % Creme.
Bei einem ausgeprägten klinischen Bild mit extrakutaner Organbeteiligung ist ein
multidisziplinäres Management erforderlich. Zukünftig könnten neue Therapieformen wie die autologe epidermale Stammzelltransplantation und gentherapeutische
Verfahren zum Einsatz kommen. Eine humangenetische Beratung ist unerlässlich. If blistering occurs in childhood, the possibility of hereditary epidermolysis bullosa should be considered even if the symptoms are mild. Besides clinical and histological examination, a molecular genetic screening is diagnostically relevant. For localized forms, symptomatic, topical therapy options are currently still the primary choice, especially interesting is the new possibility of a topical therapy with Diacerein 1 % cream. In the case of a pronounced clinical picture with extracutaneous organ involvement a multidisciplinary management is required. In the future, new forms of therapy such as autologous epidermal stem cell transplantation and gene therapeutic procedures could be applied. Human genetic counselling is indispensable. |
