Abstract: Lymphome sind Neoplasien des lymphatischen Systems, die man in Hodgkin- und Non-Hodgkin-Lymphome einteilen kann. In der Gruppe der Non-Hodgkin-Lymphome unterscheidet man des Weiteren zwischen B- und T-Zell-Neoplasien. B-Zell Lymphome bestehen aus entarteten B-Lymphozyten und haben einen heterogenen Krankheitsverlauf. Das Mantelzelllymphom gehört zu den B-Zell Non-Hodgkin-Lymphomen und macht nur etwa 6 – 9 % aller Lymphome aus. Als Ursprungszelle gilt eine periphere Antigen-naive B-Zelle aus der inneren Mantelzone eines Lymphfollikels. Die pathognomonische genetische Veränderung ist die chromosomale Translokation t(11;14), die zu einer Überexpression von Cyclin D1 führt. Die Therapie des Mantelzelllymphoms ist palliativ und stellt eine Herausforderung dar. Eine Kombination aus Chemo- und Immuntherapie stellt die Basis der Therapie dar. Durch eine Intensivierung der Induktionstherapie bei jungen, körperlich fitten Patienten kann die Prognose verbessert werden. Viele Patienten sprechen nur schlecht auf die Therapie an, weshalb die mittlere Lebenserwartung bei nur fünf Jahren liegt. Eine B-Zelle besitzt aufgrund genetischer Rekombinationsprozesse einen für sie einzigartigen B-Zell Rezeptor, der sich in seiner variablen Region von den B-Zell Rezeptoren anderer B-Zellen unterscheidet. Aufgrund des monoklonalen Ursprungs von Mantelzelllymphom Zellen besitzen alle Lymphom Zellen eines Mantelzelllymphoms einen identischen B-Zell Rezeptor. Die natürliche Aufgabe von B-Zell Rezeptoren liegt in der Erkennung von Antigenen. Da alle malignen Zellen eines Mantelzelllymphoms identische B-Zell Rezeptoren tragen, wird durch diese immer das gleiche Antigen erkannt. LRPAP1 konnte als Antigen von etwa 45% aller Mantelzelllymphome identifiziert werden. Das Ziel dieser Arbeit liegt in der Entwicklung eines Therapiekonzeptes für das Mantelzelllymphom, das über die Integrierung des Antigens LRPAP1 den B-Zell Rezeptor der Lymphom Zellen als Zielstruktur erkennt. Hierfür wird LRPAP1 als Fab-Konstrukt kloniert und exprimiert, das in seiner Form dem Antigen bindenden Fragment eines Antikörpers angelehnt ist. Anschließend wird dieses Konstrukt, das als BAR-body benannt wurde, auf spezifische Bindung an Mantelzelllymphom Zellen getestet. Über die Kopplung an eine Wirkkomponente (ETA – Toxin) wird weiterhin das therapeutische Potential des BAR-body evaluiert.
Lymphomas are neoplasias of the lymphatic system that can be divided into Hodgkin and non-Hodgkin lymphomas. In the group of non-Hodgkin's lymphomas, a distinction is also made between B-cell and T-cell neoplasia. B-cell lymphomas consist of degenerate B-lymphocytes and have a heterogeneous course of disease. mantle cell lymphoma belongs to the B-cell non-Hodgkin's lymphomas and accounts for only about 6 - 9 % of all lymphomas. The cell of origin is a peripheral antigen-naive B-cell from the inner mantle zone of a lymph follicle. The pathognomonic genetic alteration is the chromosomal translocation t(11;14), which leads to an overexpression of cyclin D1. The therapy of mantle cell lymphoma is palliative and challenging. A combination of chemo- and immunotherapy is the basis of therapy. The prognosis can be improved by intensifying induction therapy in young, physically fit patients. Many patients respond poorly to the therapy, which is why the average life expectancy is only five years. Due to genetic recombination processes, each B-cell possesses a B-cell receptor that is unique for it and differs in its variable region from the B-cell receptors of other B-cells. Due to the monoclonal origin of mantle cell lymphoma cells, all lymphoma cells of a mantle cell lymphoma have an identical B-cell receptor. The natural function of B-cell receptors is the recognition of antigens. Since all malignant cells of a mantle cell lymphoma carry identical B-cell receptors, they recognize the same antigen. LRPAP1 has been identified as the antigen of about 45% of all mantle cell lymphomas. The aim of this work is to develop a therapeutic concept for mantle cell lymphoma that recognizes the B-cell receptor of lymphoma cells as a target structure by integrating the antigen LRPAP1. For this purpose, LRPAP1 is cloned and expressed as a Fab construct, which in its form is based on the antigen-binding fragment of an antibody. Subsequently, this construct, which we designated BAR-body, is tested for specific binding to mantle cell lymphoma cells. The therapeutic potential of the BAR-body is further evaluated by coupling it to a cytotoxic component (ETA - toxin).
