Abstract: | Einleitung
Der Knochen ist ein häufiger Ort der Metastasierung und kennzeichnet bei Befall oft eine
verkürzte Prognose bei Patienten mit Mammacarcinom. Die relative Inzidenz der ossären
Metastasierung bei Patienten mit fortgeschrittenem Mammacarcinom liegt bei etwa 60%. Sie
kann kaum kurativ behandelt werden, es ist aber möglich, den Progress der Metastasierung
zu verlangsamen und die Lebensqualität der Patienten zu verbessern. Bisphosphonate,
insbesondere Zoledronsäure, sind gut bekannte Substanzen in der Therapie ossärer
Metastasierung. Eine andere Möglichkeit der Therapie ist der humane monoklonale RANKLAntikörper
Denosumab. Obwohl diese Präparate häufig verschrieben werden, sind Charakter
und Entwicklung von skeletal related events (SRE), insbesondere bei Patienten mit
Mammacarcinom, noch nicht ausreichend evaluiert. Ziel dieser Studie war es, den Effekt
beider Substanzen bei Patienten mit Mammacarcinom und ossärer Metastasierung auf die
Frequenz des Auftretens und zeitliche Entwicklung der SRE zu untersuchen.
Methodik:
Die vorliegende Arbeit ist eine retrospektiv angelegte Studie mit n=143 Patientinnen mit
histologisch nachgewiesenen Mammacarcinom und ossärer Metastasierung, welche am
Universitätsklinikum des Saarlandes in der Klinik für Frauenheilkunde, Geburtshilfe und
Reproduktionsmedizin (Deutschland) zwischen dem 1.10.2009 und dem 1.10.2013
behandelt wurden. Die Daten wurden in digitalisierten Patientenakten gesammelt. Als SRE
wurde das Vorliegen einer pathologischen Fraktur, einer ossären Operation, einer
Spinalkanalkompression aufgrund einer ossären Metastase sowie einer ossären Radiatio
definiert. Die Patientinnen wurden entsprechend der erhaltenen antiresorptiven Medikation in
fünf Behandlungsgruppen eingeteilt: Gruppe 1: Bisphosphonate (n=63 Patientinnen), Gruppe
2: Denosumab (n=55 Patientinnen), Gruppe 3: Bisphosphonate mit Wechsel zu Denosumab
(n=16 Patientinnen), Gruppe 4: Denosumab mit Wechsel zu Bisphosphonaten (n=4
Patientinnen), Gruppe 5: keine antiresorptive Therapie (n=5 Patientinnen). Ein Progress
der ossären Metastasierung ohne Vorliegen eines SRE wurde als Krankheitsprogress
definiert.
Ergebnisse:
Bei Erstdiagnose des Mammacarcinoms lag das mittlere Alter des Kollektivs bei 55,2 ± 13,1
Jahren. Bei Datenerhebung waren n=53 Patientinnen am Leben, n=83 Patientinnen
verstarben während des Follow-up. Bei n=7 Patientinnen kam es zum lost to follow up. Das
mittlere Follow-up zwischen dem Zeitpunkt der Erstdiagnose des Mammacarcinoms bis zum
Ende der Datenerhebung lag bei 88,6 ± 78,5 Monaten. Bei n=103 (72%) Patientinnen
erfolgte die Diagnose der ossären Metastasierung im Verlauf der Erkrankung. Insgesamt erhielten n=83 Patientinnen ein Bisphosphonat, während n=75 Patientinnen Denosumab
erhielten. N=20 Patientinnen erhielten sequentiell sowohl Bisphosphonate als auch
Denosumab. In dem hier gezeigten Kollektiv erlitten n=95 Patientinnen nur ein SRE.
Mindestens ein SRE erlitten 65.1% der Patientinnen aus Gruppe 1 (Bisphosphonate), 65.5%
innerhalb der Gruppe 2 (Denosumab). Die Behandlungsgruppen, die die Therapeutika
gewechselt hatten, zeigten im Verlauf die meisten SRE: Gruppe 3: 81,3%, Gruppe 4: 75,0%.
Die mittlere Therapiedauer vor Auftreten des ersten SRE lag bei 12,33 Monaten in Gruppe 1,
1,5 Monaten in Gruppe 2, 38,2 Monaten in Gruppe 3 und bei 4 Monaten in Gruppe 4. N=63
von n=95 Patientinnen erhielten nach Diagnose des ersten SRE eine Therapieanpassung.
Der Zeitraum zwischen ersten und zweiten SRE lag bei 3,2 Monaten in Gruppe 1, bei 10,4
Monaten in Gruppe 2, bei 38,7 Monaten in Gruppe 3, bei 10,0 Monaten in Gruppe 4 sowie
bei 3,0 Monaten in Gruppe 5. Die niedrigste Inzidenz von mehr als einem SRE hatten die mit
Denosumab therapierten Patientinnen (14,5%). Bei 3,6% aller Patientinnen des Kollektivs
wurde eine Kieferosteonekrose diagnostiziert, davon erhielten 2,9% der Patientinnen
Bisphosphonate, 0,7% Denosumab. Ebenfalls 3,6% des Kollektivs entwickelten eine
Niereninsuffizienz unter der antiresorptiven Therapie. Davon erhielten 2,9% der Patientinnen
Bisphosphonate, 0,7% ebenfalls Denosumab.
Schlussfolgerung:
Die in dieser Studie präsentierten Daten deuten darauf hin, dass bei Patientinnen mit ossärer
Metastasierung eines Mammacarcinoms eine antiresorptive Medikation mit Denosumab
bezüglich der Prävention des ersten SRE gleichwertig einer Therapie mit Bisphosphonaten
ist. Nach dem ersten SRE profitieren die Patientinnen von einer antiresorptiven Therapie
verglichen mit nicht therapierten Patientinnen. In diesem Kollektiv zeigten die Patientinnen
unter Denosumab die besten Resultate bei der Prävention eines zweiten SRE sowie den
niedrigsten Durchschnittswert an aufgetretenen SRE. Auch die Entwicklung von
unerwünschten Arzneimittelnebenwirkungen (UAW), wie das Auftreten einer
Kieferosteonekrose oder Niereninsuffizienz unter Therapie, sind seltener unter Denosumab
als unter Bisphosphonaten. Die häufigsten SRE waren eine nötige Radiatio einer oder
mehrerer ossärer Metastasen sowie die Kombination aus Fraktur und
Spinalkanalkompression. Introduction: Bone is a frequent site of metastases and often indicates a short-term prognosis in breast cancer patients. The relative incidence of bone metastases in patients with advanced metastatic breast cancer is about 60%. Bone metastasis can rarely be cured, but it can still be treated to slow down its growth and to improve quality of life. Bisphosphonates, especially Zoledronic acid, are well known substances in therapy of bone metastases. Another possibility in therapy is the human monoclonal RANKL-antibody Denosumab. Altough these supplements are often prescribed, the character and the development of skeletal related events (SRE) particularly in breast cancer patients are still not sufficiently evaluated. To examine the effect of both substances in patients with breast carcinoma and bone metastases on frequency and temporal proximity of SRE was the ambition of this study. Methods: This is a retrospective study of n=143 patients which includes patients with histologically proven breast cancer and bone metastasis at the department of Gynaecology and Obstetrics at the University of Saarland (Germany) between October 2009 and October 2013. Data was collected via digitized patient files. SREs were defined as a pathological fracture, a bone surgery, a spinal cord compression caused by a bone metastasis and a radiation to bone. Patients were distributed into 5 different groups according to the treatment received. Group 1: bisphosphonate (n=63 patients), Group 2: denosumab (n=55 patients), Group 3: bisphosphonate switch to denosumab (n=16 patients), Group 4: denosumab switch to bisphosphonate (n=4 patients), Group 5: no anti-resorptive therapy (n=5 patients). Progression of bone metastasis without skeletal-related event (SRE) was defined as progressive disease. Results: At the time of initial diagnosis of breast cancer the average age of collective was 55.2 ± 13.1 years. At the time of data collection n=53 patients were alive, n= 83 had died during follow up, n=7 were lost to follow up. The median follow up from the first diagnosis of the primary breast cancer and the end of data collection was 88.6 ± 78.5 month. In n=103 (72%) patients bone metastases were detected in course of disease. Overall, n=83 patients recieved an anti-resorptive therapy with bisphosphonates while n=75 patients were treated with denosumab. N=20 patients were sequentially treated with both bisphosphonates and denosumab. In the overall collective, merely one SRE occured in 95 patients. 65.1% of Group 1 (bisphosphonates) and 65.5% of Group 2 (densosumab) patients suffered from at least one SRE. The Switch groups showed the most frequent SREs: Group 3: 81.3%, Group 4: 75.0%. The median duration of treatment prior to the first SRE was 12.33 month in Group 1, 1.5 months in group 2, 38.2 months in Group 3 and 4 month in group 4. N=63 of n=95 patients received a treatment modification after the first SRE. The time period between the first and second SRE was 3.2 months in Group 1, 10.4 months in group 2, 38.7 months in group 3 10.0 months in group 4 and 3.0 months in group 5. The lowest incidence of more than one SRE was detected in the denosumab treated group (14.5%). 3.6% of patients in the overall collective were diagnosed with osteonecrosis of the Jaw, thereof 2.9% received Bisphosohonates, 0.7% Denosumab. Also 3.6% suffered from renal insufficiency while treated with antiresorptive therapy. This time 2.9% received Bisphosphonates while 0.7% received Denosumab as well. Conclusion: The data presented in this study suggests that anti-resorptive therapy with densomab is equal to the bisphosphonate therapy to prevent the first skeletal related event in patients with bone metastasis of breast cancer. After the first SRE patients benefit of an anti-resorptive therapy compared to no treatment. In our population, patients treated with denosumab showed the best results in the prevention of a second SRE and the lowest average number of SREs. Even the development of adverse events, such as osteonecrosis of the Jaw or renal insufficiency while receiving therapy are less common with denosumab than with bisphosphonates. The most frequent SRE was a radiation of one or more bone metastasis, bone fractures and the combination of bone fracture and spinal cord compression. |
