Abstract: Tumorerkrankungen sind geprägt von diversen, teils entitätsspezifischen, genomischen Veränderungen, die zur Entstehung der Tumore, zu deren Wachstum und zum Teil zur Metastasierung führen. Das Nierenzellkarzinom stellt den dritthäufigsten urologischen Tumor dar und ist in seiner Genese maßgeblich geprägt von Funktionsverlusten in Genen, die auf Hypoxie reagieren. Zusätzlich häufen Nierentumore im Verlauf ihrer Entstehung weitere Veränderungen an, die teils als „driver“ den Progress der Erkrankung vorantreiben und teils als sogenannte „passenger“ keinen direkten Vorteil für die Tumorzellen darstellen, aber insgesamt zu einer zusätzlichen Komplexität des Krebsgenoms führen. Für die, in der modernen Krebstherapie immer wichtiger werdende individualisierte, oder personalisierte Medizin, ist es notwendig diejenigen Veränderungen zu identifizieren, aus denen sich eine geeignete Therapie und/oder Nachsorge für den Patienten ableiten lassen. Da das durchschnittliche 5-Jahresüberleben durch die Ausbildung von Fernmetastasen des Nierenzellkarzinoms von etwa 90% auf nur noch etwa 10% sinkt, ist die Metastasierung einer der wichtigsten prognostischen Faktoren. Die Identifikation derer Veränderungen, die zu einem Progress der Erkrankung beitragen und damit maßgeblich die Prognose der Patienten beeinflussen, ist demzufolge enorm wichtig. Ausgehend von der genetischen Beschaffenheit eines Tumors kann daraus die Nachsorge und Therapie individuell an den einzelnen Patienten angepasst werden. In vorangegangenen array-basierten komparativen genomischen Hybridisierungen und Fluoreszenz-in-situ-Hybridisierungs-Analysen wurden genetische Aberrationen identifiziert, die mit der Metastasierung und dem Überleben von Patienten mit klarzelligen Nierenzellkarzinomen korrelierten. Die Validierung dieses genetische Scores und Identifikation sowie Analyse metastasierungsassoziierter Gene wurde in der vorliegenden Arbeit durchgeführt. Es ist gelungen den zuvor bereits publizierten genetischen Prognosescore (TNSA) an einer unabhängigen Kohorte zu validieren. Um die Aussagekraft zu verbessern und die Integration in bestehende prognostische Schemata zu ermöglichen, wurden Vergleiche des TNSA mit etablierten prognostischen Parametern durchgeführt, woraus sich ein neuer Risikoscore errechnen ließ, welcher die T-Kategorie und den genetischen Prognosescore einschloss. Dieser neue Risikoscore ist für die Stratifizierung von Patientenkohorten und für die individuelle Risikobewertung von Patienten hochsignifikant und mit einer Sensitivität und Spezifität von jeweils 88% den anderen untersuchten prognostischen Parametern überlegen. Als Vorhersagemodell für eine metachrone Metastasierung ist der Risikoscore auch in organbegrenzten Tumoren geeignet. Es konnte eine hochsignifikante Trennung der Überlebenskurven und ein über 14-fach erhöhtes Metastasierungsrisiko für Hochrisikopatienten gezeigt werden. Anhand einer zweiten Kohorte wurde der genetische Score für die Verwendung von formalinfixierten in Paraffin eingebetteten (FFPE) Gewebeschnitten etabliert. Dies erlaubte die Anwendung des neuen, integrierten Risikoscores für diese Kohorte, wodurch eine Validierung dessen erreicht werden konnte. Für organbegrenzte Tumore zeigte sich, dass der genetische Prognosescore der einzige Parameter ist, welcher signifikant mit dem Metastasierungsrisiko assoziiert ist. Der neu erstellte Risikoscore führte auch in der Validierungskohorte zu einer signifikanten Stratifizierung der Überlebenskurven mit einem über 4-fach erhöhten Metastasierungsrisiko in der Gesamtkohorte und einem über 10-fach erhöhten Risiko in organbegrenzten Tumoren für Patienten in der Hochrisikogruppe. Im zweiten Teil dieser Arbeit wurden Gene identifiziert, welche auf den veränderten chromosomalen Regionen liegen und im Metastasierungsprozess eine Rolle spielen könnten. Dazu wurden die Daten des The Cancer Genome Atlas (TCGA) ausgewertet und 12 Kandidatengene ausgewählt, von welchen sich sechs in anschließenden quantitativen Untersuchungen als signifikant dereguliert in metastasierten Tumoren erwiesen und mit einem verkürzten Überleben assoziiert waren. Einen funktionellen Einfluss auf das Überleben von Tumorzellen konnte für BCL-xL nachgewiesen werden. Die spezifische Inhibierung der BCL-xL messenger-RNA führte zur Aktivierung der Caspasekaskade in Folge dessen zur Apoptose.
Tumours are characterized by diverse and partly entity specific genomic aberrations, that lead to tumorigenesis and metastatic spread. Renal cell carcinomas are the third most common urologic cancers and are mainly characterized by the loss of function of genes involved in hypoxia signalling. In addition, renal tumours accumulate aberrations that can either, as “drivers”, promote the progression of the disease, or as so called “passengers”, not yield a direct advantage for tumour cells, but result in added complexity of the cancer genome. For the concept of individualized or personalized medicine in modern cancer therapy it is necessary to identify those aberrations from which a suitable therapy and/or a fitting follow-up for the patient can be deduced. As the mean 5-year survival rate drops from about 90% to a mere 10% through the formation of metastasis it is apparent that metastasis formation is one of the most important prognostic factors. The identification of the aberrations that cause a progress of the disease and thereby essentially influence the patient’s prognosis is consequently hugely important. Based on the genetic make-up of a tumour the follow-up and therapy can be tailored to the individual patient. In previous array based comparative genomic hybridization and fluorescent-in-situ-hybridization analyses genetic aberrations have been identified, that correlate with metastasis status and survival of patients with clear cell renal cell carcinoma. The validation of this genetic score and the identification as well as analysis of metastasis associated genes was carried out in the present work. The previously published genetic prognostic score (TNSA) was successfully validated in an independent cohort. To improve the significance of the TNSA and enable the integration of it into existing prognostic schemes, it was compared to established prognostic parameters whereby a new risk score was calculated which comprises of the T-category and the genetic prognostic score TNSA. This new risk score significantly stratifies patient cohorts and is, with a sensitivity and specificity of 88%, superior to the other analysed prognostic parameters. As a prediction model for metachronous metastasis the new risk score is also applicable in organ confined tumours. A highly significant separation of the survival curves and an over 14-fold increase in metastatic risk was demonstrated for high risk patients. Using a second cohort the analysis of the genetic score was established and validated on formalin fixed paraffin embedded tissue sections. This allowed the application of the new integrated risk score for this second cohort, whereby a validation of the score was accomplished. For organ confined tumours the genetic prognostic score (TNSA) was the only parameter significantly associated with metastasis is the second cohort. The newly created risk score also significantly stratified the survival curves in this validation cohort with a 4-fold increase in metastatic risk for the whole cohort and a 10-fold increase for patients with organ confined tumours grouped into the high-risk group. For the second part of this work genes which are located on the aberrant chromosomal regions and might play a role in the metastatic process were identified. For this purpose, the data of The Cancer Genome Atlas (TCGA) was analysed and 12 candidate genes selected of which six were shown to be significantly deregulated in metastatic tumours and associated with shortened survival in subsequent quantitative analyses. A functional influence on the survival of tumour cells was demonstrated for BCL-xL. The specific inhibition of BCL-xL messenger RNA lead to the activation of the caspase cascade an as a result to apoptosis.
