Abstract: Einleitung Bei Patienten mit operativ korrigierter Fallot´scher Tetralogie (TOF) ist zusätzlich zu dem rechten Ventrikel auch der linke Ventrikel (LV) in seiner Funktion eingeschränkt. Mittels dreidimensionaler echokardiografischer Untersuchung ist neben einer verbesserten Volumetrie auch die Bestimmung des Systolic Dyssynchrony Index (SDI) und die Analyse regionaler Strainwerte in longitudinaler, circumferentieller und radialer Richtung möglich. Methode Bei 44 asymptomatischen Jugendlichen und Erwachsenen nach Korrektur einer Fallot´schen Tetralogie und 62 altersgleichen Herzgesunden wurden echokardiografisch 3D-Datensätze in Echtzeit akquiriert. Mittels spezieller Software wurde die 3D-Ejektionsfraktion, der SDI (Systolic Dyssynchrony Index, Standardabweichung der Zeit zum Erreichen des minimalen Volumens aus 16 regionalen Myokardvolumina, Tomtec) und die longitudinalen, circumferentiellen und radialen Strainwerte in 17 Segmenten des LV (LVQ-Analyse, GE) bestimmt. Als Flächenstrain wurde das Produkt aus longitudinalem und circumferentiellem Strain berechnet. Ergebnisse Volumetrisch fand sich eine hoch signifikant erniedrigte Ejektionsfraktion und ein hoch signifikant erhöhter SDI im linken Ventrikel bei den TOF Patienten. Die globalen Strainwerte GLS (p<0.0001), GCS (p=0.006) und GRS (p=0.011) waren im linken Ventrikel bei den Patienten im Vergleich mit der Kontrollgruppe signifikant erniedrigt. Von den in der regionalen Strain-Analyse untersuchten Segmenten waren in der TOF-Gruppe 82% (2445 aus 2992 Segmenten) und in der Vergleichsgruppe 91% (3839 aus 4216 Segmenten) auswertbar. Hochsignifikant erniedrigte Strainwerte waren im radialen, longitudinalen und Flächen-Strain im Bereich mitt-anteroseptal und apikal-septal lokalisiert. Eine Untergruppierung der Patienten zeigte, dass TOF-Patienten mit schlechteren LVEF, SDI, QRS-Dauer, RVSP und bei Homograft OP zusätzlich mehr signifikant erniedrigte circumferentielle Segmente aufweisen. Schlussfolgerung Die Echtzeit-Strain-Analyse in drei Bewegungsrichtungen anhand von Volumendatensätzen ist bei Fallot-Patienten zuverlässig möglich. Die reduzierte LVEF scheint am stärksten durch topographisch am rechtsventrikulären Ausflusstrakt begrenzte Segmente bedingt zu sein. Somit scheint die Resektion von Muskelbündeln und Patcherweiterung des RVOT direkt die Funktion der linken Kammer im Langzeitverlauf zu beeinträchtigen. Der GLS ist der aussagekräftigste globale Strain zur Detektion des Deformationsdefizits des LV bei Patienten nach Fallotkorrektur. Der circumferentielle Vektor macht latente Störungen des LVMyokards sichtbar.
3D-strain and dyssychrony-analysis of the left ventricle in patients with surgically repaired tetralogy of Fallot Indroduction In patients with surgically repaired tetralogy of Fallot (TOF) the left ventricle (LV) is reduced in its function. Threedimensional echocardiography offers not only the volumetric analysis, but also the determination of the Systolic Dyssynchrony Index (SDI) and the analysis of the regional Strain in longitudinal, circumferential and radial direction. Methods The study included 44 asymptomatic juveniles and adults after repair of tetralogy of Fallot and 62 healthy controls of the same age, who had 3Dechocardiography in real-time. The 3D ejection fraction, the SDI (Systolic Dyssynchrony Index, standard deviation of the duration to reach the minimal volume in 16 regional myocardial segments, Tomtec) and the regional strains of the 17 LVsegments (LVQ-analysis, GE) were determined within a single beat three dimensional dataset. The product of longitudinal and circumferential strain was calculated as area strain. Results 3D volumetric analysis revealed a highly reduced LV-ejection fraction in addition to significant elevated SDI in the TOF patients in comparison to healthy controls. The global strain parameters GLS (p<0.0001), GCS (p=0.006) and GRS (p=0.011) were significantly reduced in LV. Regarding the regional strain analysis in the TOF-patients 82% (2445 out of 2992 segments) and 91% (3839 out of 4216 segments) in the control group were adequate for analysis. Highly significant reduced regional strains were localized in radial, longitudinal and area strain in the midanteroseptal und apical-septal region. By dividing the patients in subgroups in regard to LVEF, SDI, QRS-duration and type of surgery, regional circumferential strain values were able more adequately to identify the TOF patient with altered LV function. Conclusion 3D echo analysis of the LV accompanied with 3D with volumetric measurements of LV systolic and diastolic volumes are reliable to determine the global and regional myocardial function in patients after TOF repair. The GLS has the highest validity for detection of deformation deficits in LV, compared to other global strains. The circumferential strain reveals latent regional LV-myocardial dysfunction, which seems to be related to the corrective surgery in the right ventricular outflow tract.
