Abstract: | Die knöcherne Verletzungen des Beckenrings ist eine seltene Frakturen des
Menschen. Nur ca. 2%–8% aller Frakturen betreffen den Beckenring, wobei instabile
Beckenringfrakturen hierbei eine erhöhte Mortalität aufweisen. Ursächlich hierfür ist
ein hoher Energiefluss um den knöchernen Beckenring zu durchbrechen, welcher
zusätzliche und potenziell tödliche Begleitverletzungen verursacht. So sind bei
schweren Beckenverletzungen häufig intraabdominelle Organe, wie die Harnblase,
das Colon aber auch die im Becken verlaufenden Gefäße betroffen. Die Fraktur
begleitende, intrapelvine Massenblutung ist hierbei die führende Todesursache.
Blutungen aus den Iliacalgefäßen sowie dem präsakralem Venenplexus und dem
Knochen selbst führen rasch zu einer hämodynamischen Instabilität des Patienten.
Kenntnisse über den Unfallmechanismus, sowie klinische und radiologische
Primärbefunde sind unerlässlich zur Primärevaluation und geben erste Hinweise über
das Ausmaß der Verletzung sowie die notwendige Therapie. Dennoch bleibt die
Einschätzung des potenziellen Blutverlustes häufig schwierig und folgt keinen
regulierten Standards. Zur Abschätzung des akuten Blutverlustes werden vorrangig
klinische Zeichen des Schocks verwendet welche jedoch häufig erst zeitverzögert
Rückschlüsse auf den Blutverlust zulassen, da Kompensationsmechanismen des
menschlichen Körpers ein imminentes Schockgeschehen überdecken.
Weitere Möglichkeiten, intrapelvine Blutverluste aufzudecken, bieten die
Sonographie, die Computertomographie sowie die Angiographie. Diese Verfahren
lassen konkretere Abschätzungen über das Ausmaß des Blutverlustes zu, benötigen
jedoch eine eingehende Evaluation und Bearbeitung, um detaillierte Rückschlüsse auf
die Blutungsmenge zu geben. Ein Verfahren zur Volumenberechnung anhand von CT-Datensätzen ist die
sogenannte CT-Volumetrie. Mit ihr kann durch Flächenmarkierung der
Blutungsareale in den einzelnen Schnittbildern ein Volumen berechnet werden.
Die vorliegende Arbeit demonstriert, wie unter Zuhilfenahme des Verfahrens der CTVolumetrie
der Blutverlust bei instabilen Beckenverletzungen rekonstruiert und somit
eine reelle Abschätzung der Blutungsmenge ermöglicht wird. Untersucht wurde
zudem eine mögliche Korrelation des Frakturmusters nach AO/OTA
(Arbeitsgemeinschaft für Osteosynthesefragen / Orthopaedic Trauma Association) mit
der Blutungsmenge, zur Ermittlung des Frakturtyps als potenziellen Prädiktor für die
Größenordnung der verletzungsbegleitenden Blutung. Somit könnte bereits die initiale
Röntgendiagnostik, welche zur Standardprozedur der Schockraumdiagnostik gehört,
den behandelnden Ärzten bereits nach Frakturklassifikation wertvolle Hinweise auf
das Ausmaß der zu erwartenden Blutungsmenge sowie der daraufhin einzuleitenden
weiteren Diagnostikmaßnahmen und Therapieschritte geben. Volume determination of pelvic blood loss in polytrauma patients with an unstable pelvic fracture Fractures of the pelvic ring are rare, accounting for 2%-8% of all fractures. In particular unstable pelvic ring fractures go along with an increased mortality. The force necessary to disrupt the pelvic ring often causes additional damage to surrounding tissues including internal organs and vessels. Mass bleeding within the pelvis is the leading cause of mortality in patients with pelvic ring fractures. Common sources of bleeding are arterial bleeds in the iliac basin, the presacral venous plexus and cancellous bone surfaces, which can rapidly result in haemodynamical instability. The history of the mechanism of trauma as well as the clinical and radiological findings, are essential for primary evaluation, providing information regarding the extent of the injury and the treatment needed. However, the assessment of the amount of primary blood lost into the pelvic cavity is difficult and does not follow any standard evaluation procedure. In order to acutely assess the volume of total blood loss and haemodynamic status of the patient, clinical shock signs such as systolic blood pressure (SBP) and haemoglobin (Hb) concentration are utilized. However, homeostatic compensatory mechanisms may disguise an impending shock with normotension and initially normal haemoglobin values. Alternative diagnostic modalities are able to determine the amount of bleeding. Ultrasound, computed tomography (CT) and angiography can visualize and estimate the extent of bleeding, but exact volume measurements can only be obtained with further computed reconstruction. CT-Volumetry reconstructs a volume by measuring the areas of interest in each sectional image. Using CT-Volumetry to reconstruct the amount of blood loss in patients with unstable pelvic ring fractures, this thesis investigates a correlation between the type of pelvic fracture (AO/OTA) and the bleeding volume in order to utilize the fracture type as a potential predictor for the magnitude of its concomitant bleeding. As a consequence initially taken x-ray images, which are part of the standard trauma investigational protocol, may already provide treating physicians with valuable information about the magnitude of the forthcoming blood loss and guide us in initiating further diagnostics and life-saving therapy in a timely manner. |
