Abstract: | Die Chronisch Obstruktive Lungenerkrankung (COPD) steht weltweit im Fokus der
medizinischen Forschung, da sie nicht nur zu den häufigsten Todesursachen zählt, sondern
auch keine kausale Therapie verfügbar ist. Bisher gelang es nicht verlässliche
diagnostische oder prognostische Biomarker zu finden, welche die Therapie und die
Diagnostik der COPD unterstützen.
Das Ziel dieser Arbeit war die Analyse neuer Biomarker für die Verwendung bei der
COPD. Dazu untersuchte die Kohortenstudie des German COPD and Systemic Consequences
– Comorbidities Network (COSYCONET) mit Hilfe von Immunassays die
Serumproben von 2741 Patienten. Der Fokus lag auf den Biomarkern Angiopoetin-like
Protein 4 (ANGPTL4), Growth differentiation factor 15 (GDF-15) und N-terminales pro
brain natriuretic peptide (NT-proBNP). Ziel der COSYCONET-Studie war es Informationen
zu Komorbiditäten, subjektiven Symptomausprägungen und den Ergebnissen
klinischer Untersuchungen zu sammeln, mit Hilfe derer die Korrelation zwischen den
Serumkonzentrationen der analysierten Proteine und den klinischen Parametern im
Querschnitt oder der longitudinalen Analyse bestimmt werden kann.
Nach dem Erstellen einer deskriptiven Statistik, um einflussreiche Störfaktoren zu ermitteln,
wurde unter Berücksichtigung dieser eine multiple Regressionsanalyse durchgeführt.
Die ANGPTL4 Konzentration korreliert mit den Lungenfunktionsparametern des spezifischen
effektiven Atemwegswiderstands (sRAWeff.), des Residualvolumens (RV) und
des intrathorakalen Gasvolumens (ITGV). Zusätzlich fand sich ein Zusammenhang zwischen
ANGPTL4 und der Lebensqualität gemessen mit Hilfe des St.George Respiratory
Questionnaire (SGRQ) und des 6-Minuten Gehtests (SMWD). Des Weiteren korreliert die
Konzentration mit den Laborparametern des CRPs (CRP) und des HDL-Cholesterins
(HDL). Eine weitere Assoziation zeigt sich zwischen dem klinisch erhobenen BODE-Index
und der Biomarker-Konzentration.
Bei dem gemessenen Biomarker GDF-15 ergab sich eine signifikante Assoziation mit
der Diffusionskapazität für Kohlenmonoxid (DLCO). Zusätzlich bestehen Zusammenhänge
mit den Komorbiditäten Fibrose, Sarkoidose, Herzinfarkt, arterielle Hypertonie,
Venenthrombose und Diabetes mellitus. Auch dieser Biomarker korreliert mit der Lebensqualität
– gemessen mit Hilfe des SGRQ und des SMWD. Der Ankle Brachial Index (ABI), der zur Abschätzung des kardiovaskulären Risikos dient, weist einen Zusammenhang
mit der GDF-15 Konzentration auf.
Die NT-proBNP-Konzentration zeigt keine Korrelation zu Lungenfunktionsparametern. In
der multivariablen Regressionsanalyse konnten erhöhte NT-proBNP-Werte bei Patienten
mit den Komorbiditäten koronare Herzkrankheit (KHK), Schlaganfall und Herzinsuffizienz
gefunden werden.
Zusammenfassend kann gesagt werden, dass es schwierig ist bei COPD Patienten einen
lungenspezifischen, wie stabilen Biomarker zu finden. Zum einen beruht dies auf der
Tatsache, dass COPD Patienten an vielen Komorbiditäten leiden. Es ist schwierig zu
differenzieren, ob die COPD die primäre Ursache der erhöhten Serumkonzentration darstellt,
oder die Komorbidität. Zweitens ist die COPD als sehr variable Krankheit bekannt,
wodurch sich ein einheitlicher Biomarker nur schwerlich finden lässt. Als dritter Punkt
ist zu nennen, dass in dieser Arbeit ein Querschnitt der COSYCONET Studie betrachtet
wird, anhand dessen keine Aussage zum Verlauf der Biomarker Konzentration getroffen
werden kann.
Insgesamt zeigt sich, dass Serumproteine sinnvolle Biomarker zur Beurteilung der COPD
sein können. The chronic obstructive pulmonary disease (COPD) is focused in medical research worldwide, as it is one of the leading causes of death and there is no causal therapy available. So far, it has not been possible to find reliable diagnostic and sparse prognostic laboratory parameters that support the therapy and diagnosis of COPD. The topic and aim of this research is finding new biomarkers for patients with COPD. Therefore, the German COPD and Systemic Consequences – Comorbidities Network (COSYCONET) study includes a large cohort with 2741 patients form which we took serum blood samples. Using immunoassays we analyzed the serum proteins angiopoetinlike- protein 4 (ANGPTL-4), growth/differentiation factor 15 (GDF-15) and N-terminal prohormone of brain natriuretic peptide (NT-proBNP). The aim of this research was to collect information about comorbidities, subjective symptom manifestations and the results of clinical examinations, which can be used to determine the correlation between the serum concentrations of the analyzed proteins and the clinical parameters in cross-sectional or longitudinal analysis. Afterwards we analyzed statistically the measured values regarding individual confounder for every biomarker. 2 1.1 Abstract We could find correlations between ANGPTL4 and the parameters of lung functions specific effective airway resistance (sRAWeff.) , residual volume (RV) and intrathoracic gas volume (ITGV). Additional correlations could be found between the ANGPTL4 concentration and the quality of life as measured by the St.George’s Respiratory Questionnaire (SGRQ) and the 6-minute walk (SMWD). Furthermore, the concentration correlates with the laboratory parameters of C-reactive protein (CRP) and HDL cholesterol. Another association shows up between the clinically raised BODE index and ANGPTL4. The measured biomarker GDF-15 showed a significant association with the carbon monoxide diffusion capacity (Diffusionskapazität für Kohlenmonoxid (DLCO)). In addition, there are correlations with the comorbidities fibrosis, sarcoidosis, heart attacks, arterial hypertension, venous thrombosis and diabetes mellitus. This biomarker also correlates with the quality of life – measured with the aid of SGRQ and the SMWD. The anklebrachial index (ABI), which is used to estimate the cardiovascular risk, has a correlation with the GDF-15 concentration. There is no correlation between the NT-proBNP concentration and pulmonary function parameters. In the multivariable regression analysis, the comorbidities coronary heart disease, strokes and heart failure remained significant. To summarize, it can be said that it is difficult to find a stable lung specific biomarker for patients with COPD. On the one hand, this is due to the fact that COPD patients suffer from many comorbidities. It is difficult to differentiate whether COPD is the primary cause of increased serum concentration or comorbidity. On the other hand, COPD is known to be a very variable disease, making it difficult to find a consistent biomarker. Additionally, this thesis looks at a cross-sectional study that does not allow conclusions to be drawn about the course of biomarker concentration. Overall, it is shown that serum proteins can be useful biomarkers for assessing patients with COPD. |
