Abstract: | Hintergrund und Ziele: Der ileoanale Pouch gilt als Goldstandard für Patienten mit
Indikationen für eine Proktokolektomie. Ziel der vorliegenden Arbeit war es, den Einfluss
verschiedener operativer Parameter auf den Kurz- und Langzeitverlauf sowie das
Pouch-Überleben und die Lebensqualität über die Zeit von drei Jahrzehnten zu
untersuchen und damit die Eignung des ileoanalen Pouches als andauernde Versorgung
nach Proktokolektomie kritisch zu überprüfen.
Patienten und Methoden: In einem ersten Teil wurden retrospektiv die Krankenblätter
von Patienten mit IAP-Operationen aus der Zeit vom 01.06.1986 bis zum 31.12.2015
analysiert. In einem zweiten Teil wurde mithilfe des standardisierten WHOQOL-BREFund
eines individuellen Fragebogens die Lebensqualität untersucht.
Ergebnisse: Das Kollektiv von 119 Patienten setzte sich aus 67 (56,3%) Männern und
52 (43,7%) Frauen zusammen, die zu 70,6% (n = 84) an einer chronisch-entzündlichen
Darmerkrankung und zu 29,4% (n = 35) an einer nicht chronisch-entzündlichen
Darmerkrankung litten. Das Alter zum Zeitpunkt der IAP-Operation betrug 33,6 ± 10,9
Jahre. Bei 90,5% (n = 105) erfolgte der Eingriff elektiv, bei 43,7 % (n = 52) als One-
Stage-Operation und bei 42,9% (n = 51) wurde eine Loop-Ileostomie angelegt. Bei
Patienten ohne Loop-Ileostomie wurde ein transanaler Katheter zur postoperativen
Anastomosenprotektion eingelegt. In 95,8% (n = 114) wurde ein J-Pouch angelegt, in
84% (n = 100) erfolgte die anale Anastomosenkonstruktion direkt. Dabei war die
Operationszeit mit 231,3 ± 73,9 Minuten signifikant (p = 0,001) kürzer als bei indirekter
Konstruktionstechnik. Der stationäre Aufenthalt dauerte durchschnittlich 26,3 ± 12,3
Tage. Zum Zeitpunkt der Entlassung betrug die tägliche Stuhlfrequenz 3,9 ± 1,6 Stühle.
Bei 14,3% (n = 17) der Anlage-Operationen traten intraoperativ sofort korrigierfähige
Komplikationen auf. 15,9% (n = 19) der Patienten erlitten postoperativ revisionspflichtige
Major-Komplikationen, wobei 7,5% (n = 9) der Komplikationen resektionsbedingt und
10,1% (n = 12) rekonstruktionsbedingt auftraten. Die Letalität betrug 0,8% (n = 1). Im
Langzeitverlauf traten bei 42 von 118 Patienten (35,6%) Spätkomplikationen auf, die in
36 Fällen (30,5%) eine chirurgische Intervention indizierten. Die Korrektur-Indikationen
waren hauptsächlich entzündliche (12,7%, n = 15) und funktionelle (16,9%, n = 20)
Komplikationen. Die Pouchitis-Inzidenz betrug 8,5% (n = 10). Die kumulativen
Wahrscheinlichkeiten erreichten über die gesamte Beobachtungszeit ein Maximum von
28,2% für entzündliche, 32,1% für funktionelle Komplikationen und 24,6% für die
Pouchitis. Zum Versagen des Pouches führten entzündliche Komplikationen in 66,7% (n
= 14), funktionelle Komplikationen nur in 28,8% (n = 6) der Fälle. Die rohe Versagensrate betrug 13,6% (n = 16), wobei die kumulative Überlebensrate bis zum 15. postoperativen
Jahr auf 72,9% sank und danach konstant blieb.
Die Lebensqualität wurde subjektiv als zufriedenstellend bewertet, objektiv wurde jedoch
nicht das Niveau einer gesunden Population erreicht. Eine signifikante subjektive
Verbesserung nach der Operation wurde bei der Ausübung des Berufs (p < 0,001), der
Belastbarkeit beim Sport (p = 0,002) sowie der Möglichkeit, Reisen zu unternehmen (p
< 0,001), festgestellt. 82,1% (n = 23/28) der Patienten würden sich heute wieder für einen
IAP entscheiden.
Diskussion und Schlussfolgerung: Die postoperativen Kurz- und Langzeitergebnisse
dieser Studie einschließlich des Pouch-Überlebens und der Lebensqualität waren mit
der internationalen Literatur vergleichbar. Gleichwohl führte die Untersuchung zu
differenzierten und teilweise von der Literatur abweichenden Bewertungen. So ergab
sich in unserem Kollektiv für die One-Stage-Anlagen die höchste perioperative, aber die
niedrigste Langzeitmorbidität. Dabei muss aber berücksichtigt werden, dass die
Langzeitkomplikationsraten stärker durch die Anastomosentechnik als durch das
operative Vorgehen beeinflusst wurden. Die postoperative Morbidität bei Patienten mit
und ohne Loop-Ileostomie kann als ähnlich bewertet werden. Da die Ileostomie-
Verschluss-Operation zusätzlich eine eigene Morbidität aufweist, muss die
Gesamtmorbidität bei Patienten mit Loop-Ileostomie als höher bewertet werden. Ein
ausreichender postoperativer Schutz der Anastomose kann andererseits bereits über
einen transanal eingelegten Katheter erreicht werden. Darüber hinaus konnte bestätigt
werden, dass der direkten Konstruktionstechnik aufgrund geringerer Komplikationsraten
und besserer funktioneller Ergebnisse der Vorzug gegeben werden sollte. Es konnte
kein erhöhtes Risiko für Krankheitspersistenz oder -rezidive bei direkten Anastomosen
nachgewiesen werden. Die Grunderkrankung stellte sich als bedeutender Einflussfaktor
für die Langzeitergebnisse heraus: entzündliche Komplikationen traten signifikant
häufiger bei Patienten mit chronisch-entzündlichen Darmerkrankungen auf und führten
zudem häufiger zum Pouch-Versagen als funktionelle Komplikationen. Somit stellt die
Grunderkrankung ein größeres Risiko als die anderen operativen Parameter dar. Auf
Grundlage dieser Ergebnisse muss geschlussfolgert werden, dass die Pouch-Operation
nicht vorbehaltslos als sichere und vor allem nachhaltige chirurgische Versorgung
betrachtet werden kann. Gleichwohl führt die Anlage auch nach unseren Ergebnissen
zur subjektiven Verbesserung der Lebensqualität im Bereich der Funktion,
Berufsausübung, Freizeitgestaltung und des Sexuallebens gegenüber dem Zustand vor
der Kolektomie. Objektiv kann allerdings nicht die Lebensqualität einer gesunden
Population erreicht werden. Background and aims: The ileoanal pouch procedure is claimed to be the operation of choice for patients undergoing proctocolectomy. This study examines the influence of different operative parameters on short-term and long-term outcome as well as the pouch-survival-rates and quality of life. Hence the adequacy of the IAP as a save and long-lasting operative treatment has been examined critically. Patients and methods: This study was both a retrospective study of all patients who underwent a restorative proctocolectomy between 01.06.1986 and 31.12.2015 and quality of life analysis by the WHOQOL-BREF-questionnaire and an individual questionnaire. Results: 119 patients have been examined of whom 67 (56,3%) were men and 52 (43,7%) women. 70,6% (n = 84) had the diagnosis of chronic inflammatory diseases and 29,4% (n = 35) of non chronic inflammatory diseases. The average age at operation was 33,6 ± 10,9 years. 90,5% (n = 105) of the indications for proctocolectomy were elective. 43,7% (n = 52) of operations were performed as One-stage-procedures and 42,9% (n = 51) of the anastomoses were covered with a protective stoma. All patients without protective stoma were provided with a transanal catheter for the postoperative protection of the anastomosis. 95,8% (n = 114) of the pouches were constructed as J-pouches, 84% (n = 100) of the anastomoses were constructed with doublestapling devices. The average duration of the operation was significantly shorter when the doublestalplingtechnique was performed (231,3 ± 73,9, p = 0,001). The average duration of hospital stay was 26,3 ± 12,3 days. After the hospital discharge the average stool frequency was 3,9 ± 1,6 times per day. Intraoperative complications were found in 14,3% (n = 17) of cases and were treated immediately. Postoperative complications leading to another operation were found in 15,9% (n = 19) of patients. The cause for re-operations was either due to resectionalcaused (7,5%, n = 9) or reconstructive-caused (10,1%, n = 12) complications. Lethality was low (0,8%, n = 1). Long-term complications were found in 42 of 118 patients (35,6%), in 36 cases (30,5%) a re-operation was indicated due to inflammatory (12,7%, n = 15) or functional (16,9%, n = 20) cause. Pouchitis was found in 10 patients (8,5%). The cumulative risk reached a maximum at 28,2% for inflammatory, 32,1% for functional and 24,6% for Pouchitis complications after three decades. Pouch-failure occurred in 16 cases (13,6%). Inflammatory complications had a higher risk to lead to pouch-failure (66,7%, n = 14) than functional complications (28,8%, n =6). The cumulative pouchsurvival dropped to a minimum of 72,9% until the fifteenth postoperative year. Quality of life was rated satisfactory in the subjective/individual analysis but did not reach the quality of life of a healthy population in the objective analysis. Subjectively, patients claimed to have significantly better results after restorative proctocolectomy for performing work (p < 0,001), sports (p = 0,002) and travel (p < 0,001). 82,1% (n = 23/28) of patients would choose the IAP as treatment after proctocolectomy again. Conclusion: The results of the study were comparable to the results of the literature. Nonetheless the examination lead to different conclusions for the adequacy of the ileoanal pouch procedure as a save and long-lasting operative treatment. One-stageprocedures were found to be the operations with the highest postoperative complication rates, but with lowest long-term complications. These results were affected more by the technique of anastomosis-construction than the staging of the operation. The postoperative morbidity in patients with and without a protective stoma was similar. Patients with a protective stoma suffered additionally from complications related to closure of the stoma leading to higher over-all complications-rate in patients with a protective stoma. A sufficient protection of the anastomosis can be provided with the postoperative placement of a transanal catheter. The doublestapling-technique leads to better results in function and lower complication rates so that it should be the technique of choice. According to literature there is a higher risk of disease persistence or recurrence in double-stapled anastomoses that could not be confirmed due to this study. The disease was found to have a strong influence on the long-term outcome: inflammatory complication rates are significantly higher in patients with chronic inflammatory diseases and lead to a failure of the pouch in two thirds of the affected patients. The disease is a stronger risk factor for complications and pouch-failure than the other operative parameters. Furthermore, the quality of life of a healthy population is not achievable, even though a subjective improvement of the life quality after the IAPoperation is reported. Due to a high morbidity and failure-rate the suitability of restorative proctocolectomy as a long-lasting procedure and an operation of choice for patients requiring a proctocolectomy must be evaluated with caution. |
