Abstract: Abstract Background: Gradually vitamin B12 deficiency is common in elderly people, but can be prevented by in-creased vitamin intake. Aims of the study: This study examined the ability of vitamin B12-enriched toothpaste to increase vitamin B12 status in elderly patients compared to a placebo. Materials and methods: A Random, double-blind, placebo-controlled treatment was performed on 103 elderly volun-teers over a three-month period. Of these, 92 met the inclusion criteria and completed the study. Serum concentrations of vitamin B12, holotranscobalamin (holoTC), methylmalonic acid (MMA), and total homocysteine (tHcy) were measured at baseline and after three months. Results: After treatment, mean levels of vitamin B12 were significantly higher in vitamin group com-pared to placebo group [mean (SD) 368 (123) vs. 295 (123) pmol/l; p = 0.005]. Concentration of HoloTC tended to increase [112 (48) vs. 91 (68) pmol/l; p = 0.088]. Changes of serum vit-amin B12 [54 (74) vs. -3 (60) pmol/l, p < 0.001, HoloTC 21 (34) vs. 2 (32) pmol/l, p = 0.007, and tHcy -0.9 (2.3) vs. 0.3 (1.9) μmol/l, p = 0.010] were significantly different between vitamin and placebo groups. Mean percentage increase in serum vitamin B12 (% 17) in vitamin B12 toothpaste group indicates that the treatment provided an additional daily intake of 7 μg oral vitamin B12. Diseases and drugs that might be associated with low vitamin B12 status did not affect the change of blood markers in the vitamin group. Conclusions: The toothpaste enriched with vitamin B12 has increased vitamin B12 status in elderly people and can be used to improve vitamin status or prevent its deficiency. Zusammenfassung Hintergrund: Ein schleichender Vitamin-B12-Mangel ist bei älteren Personen häufig, kann aber durch eine erhöhte Vitaminzufuhr verhindert werden. Ziel: In der vorliegenden Studie sollte die Fähigkeit von mit Vitamin B12 angereicherter Zahnpasta im Vergleich zu einem Placebo untersucht werden, den Vitamin-B12-Status bei älteren Patienten zu erhöhen. Materialien und Methoden: Es wurde eine stichprobenartig ausgewählte, doppelblinde, placebokontrollierte Behandlung bei 103 älteren Probanden durchgeführt, von denen 92 die Einschlusskriterien erfüllten und die dreimonatige Studie abschlossen. Die Serumkonzentrationen von Vitamin B12, HoloTC, MMA und tHyc im Plasma wurden zu Studienbeginn und nach drei Monaten gemessen. Ergebnisse: Nach der Behandlung waren die mittleren Konzentrationen von Vitamin B12 in der Vitamin-B12-Gruppe im Vergleich zur Placebogruppe signifikant höher [368 (123) pmol/l vs. 295 (123) pmol/l; p = 0,005]; die HoloTC-Konzentration war tendenziell erhöht [112 (48) pmol/l vs. 91 (68) pmol/l; p = 0,088]. Die Veränderungen von Serum-Vitamin B12 [54 (74) pmol/l vs. -3 (60) pmol/l, p < 0,001], HoloTC [21 (34) pmol/l vs. 2 (32) pmol/l, p = 0,007] und tHcy [-0,9 (2,3) μmol/l vs. 0,3 (1,9) μmol/l, p = 0,010] waren zwischen der Vitamin- und der Placebogruppe signifikant unterschiedlich. Der mittlere prozentuale Anstieg von Serum-Vitamin B12 (17 %) in der Vitamin-B12-Zahnpastagruppe deutet darauf hin, dass die Behandlung eine zusätzliche tägliche Zufuhr von 7 μg oralem Vitamin B12 bereitgestellt hatte. Krankheiten und Medikamente, die mit einem niedrigen Vitamin-B12-Status in Verbindung gebracht werden könnten, beeinflussten die Veränderung der Blutmarker in der Substitutionsgruppe nicht. Schlussfolgerung: Die mit Vitamin B12 angereicherte Zahnpasta hat bei älteren Menschen den Vitamin-B12-Status erhöht und kann somit zur Verbesserung des Vitamin-Status oder zur Vorbeugung eines Vitamin-B12-Mangels eingesetzt werden.
