Abstract: Da sich regionale Genexpressionunterschiede bei Patienen mit Aortenklappenanoma-lien ergeben haben, welche sich nach aktuellem Wissensstand nicht durch eine erhöhte Scherspannung und der damit nachgeschalteten Signalkaskaden erklären lassen, liegt es nahe, dass als Ursache der Aneurymen ein genetischer Pathomechanismus heran-zuziehen ist. Beteiligt hierbei sind der eNOS-Signalweg und der TGFß-Signalweg, welche sich auch untereinander beeinflussen. Pharmakologische Eingriffe in diese Signalkaskaden könn-ten einen therapeutischen Ansatz bieten. Genauere molekularbiologische Interaktions-kaskaden sind Bestandteil aktueller Forschungsprojekte
Summary Gene expression in the dilated ascending aorta in patients with different aortic valve abnormalities Background Unicuspid and bicuspid aortic valves are congenital cardiac anomalies predisposing to ascending aortic aneurysm. For BAV there is evidence that gen expression in the as-cending aorta differ from patients with tricuspid aortic valves. Less is known on the aor-topathy associated with UAVs. Objective Are there differences in gene expression in the aortic wall of patients with aortic valve abnormalities compared to patients with normal aortic valves? To elucidate possible pathomechanisms, the expression of genes of the eNOS (GATA5, eNOS) and TGF-β signalling pathway (FBN1, TGFBR1) as well as genes associated with thoracic aortic aneurysms (COL3A1) were investigated. Material and Methods Samples from 107 patients undergoing aortic or aortic valve surgery (48 TAV, 38 BAV, 21 UAV) were harvested intraoperatively from different locations of the ascending aor-tic wall. Specimens were obtained close to the sinotubular junction at convexity (A1) and concavity (A2) of the aorta. In aneurysm specimens were also obtained from the convexity (B1) and concavity (B2) of the mid-ascending aorta. Patients with tricuspid aortic valves and normal ascending aortic diameter served as controls. In order to find correlations mRNA expression was determined by quantitative real-time polymerase chain reaction to assess gene expression levels. Results Patients with tricuspid aortic valves and normal ascending aorta show no regional dif-ferences in gene expression of GATA5, eNOS, TGFBR1, FBN1 and COL3A1 eNOS in the ascending aortic wall. However patients with aortic valves anomalies and aortic dilatation showed significant differences in regional gene expressions. Also patients with tricuspid aortic valves and aortic aneurysm showed differences in genexpression-design, compared to those with valve anomalies. For example, patients with tricuspid aortic valves and diladed ascending aorta showed a significantly higher genexpression of eNOS in the concavity adverse the convexity of the acending aortic wall. 10 Conclusion Since regional differences in gene expressions have resulted in patients with aortic valve anomalies, which, according to the current state of knowledge, can not be ex-plained by an increased shear stress and the subsequent signal cascades. Thus, it is obvious that a genetic pathomechanism is the cause of the aneurysms. This mechanism leads to a dysregulation of the eNOS signal path and the TGFß signal path. It can be supposed, as alsready know in literature, that both influence each other. Pharmacological interventions in these signaling cascades could provide a therapeutic approach. For a save appicaltion, more detailes about the molecular biology interaction cascades are needed.
Zusammenfassung Hintergrund Die bicuspid und die unicuspid angelegte Aortenklappe sind seltene angeborene Anomalien, die im Zusammenhang mit Aneurysmen der Aorta ascendens stehen. Es gibt Anhaltspunkte, dass sich die Genexpression in der Aortenwand von Patienten mit bicuspid oder unicuspid angelegter Aortenklappe im Vergleich zu Patienten mit normal angelegter Aortenklappe unterscheiden. Bis dato ist der Pathomechanismus nur ansatzweise verstanden. Noch weniger ist über den Zusammenhang zwischen unicuspid angelegter Aortenklappe und Aneuryma der Aorta ascendens bekannt. Fragestellung Somit ist zu klären, ob Unterschiede in der Genexpression in der Aortenwand von Patienten mit Aortenklappenanomalien im Vergleich zu Patienten mit normal angelegten Aortenklappen existieren? Um mögliche Pathomechanismen zu eruieren, untersuchten wir die Expression von Genen des eNOS- (GATA5, eNOS) und TGF-β-Signalweges (FBN1, TGFBR1) sowie von Genen, die mit thorakalen Aortenaneurysmen assoziiert sind (COL3A1). Material und Methoden Von 107 Patienten (TAV 48, BAV 38, UAV 21), welche an der Aortenklappe und/oder der Aorta ascendens operiert wurden, konnten intraoperativ von verschiedenen Lokalisationen der Aorta ascendens Gewebeproben entnommen werden. Die Kontrollgruppe bestand aus Patienten mit tricuspid angelegter Aortenklappe und normalkalibriger Aorta ascendens. Die Bestimmung der Genexpression und möglicher Korrelationen erfolgte über die Isolation der RNA, anschließend die reverse Transkription in cDNA sowie schließlich die quantitative real-time PCR. Bestimmt wurde somit die Expression der mRNA für die untersuchten Gene. Ergebnisse Bei Patienten mit tricuspider Aortenklappe und normaler Aorta ascendens zeigten sich in der Genexpression in der Aortenwand von GATA5, eNOS, TGFBR1, FBN1 und COL3A1 keine regionalen Unterschiede. Hingegen konnten wir nachweisen, dass Patienten mit Aortenklappenanomalien und Dilatation der Aorta ascendens signifikante 8 Unterschiede in der regionalen Genexpression aufwiesen. Auch bei Patienten mit tricuspider Aortenklappen und Dilatation der Aorta ascendens zeigten sich signifikante Unterschiede in den lokalen Genexpressionsmustern, welche sich zudem von Patienten mit Aortenklappenanomalien unterscheiden. So sieht man z.B. bei Patienten mit tricuspid angelegter Aortenklappe und Aortenaneurysma eine signifikant höhere Genexpression von NOS3 in der Konkavität der Aorta ascendens im Vergleich zur Konvexität. Schlussfolgerung Da sich regionale Genexpressionunterschiede bei Patienen mit Aortenklappenanomalien ergeben haben, welche sich nach aktuellem Wissensstand nicht durch eine erhöhte Scherspannung und der damit nachgeschalteten Signalkaskaden erklären lassen, liegt es nahe, dass als Ursache der Aneurymen ein genetischer Pathomechanismus heranzuziehen ist. Beteiligt hierbei sind der eNOS-Signalweg und der TGFß-Signalweg, welche sich auch untereinander beeinflussen.
