Abstract: Malignome gehören mit ungefähr 8 Millionen Todesfällen pro Jahr zu den häufigsten natürlichen Todesursachen weltweit. Viele Tumorerkrankungen, wie das Pankreaskarzinom und das Nicht-kleinzellige Lungenkarzinom, zeigen erst spät Symptome und werden deswegen oft nicht rechtzeitig erkannt. Eine frühe Diagnostik ist jedoch ausschlaggebend für den Erfolg der Therapie und das Überleben der Patienten. Effiziente diagnostische Verfahren zur Vorsorge und Früherkennung von malignen Neoplasien werden daher dringend gebraucht. Einen neuen Ansatz stellt die Isolation tumorassoziierter Mikropartikel (taMP) dar, welcher in dieser Studie vorgestellt wird. Mikropartikel sind von der Zellmembran abgeschnürte Vesikel, die während Stress der Zelle, Apoptose oder bei einem Anstieg der Ca2+-Konzentration in der Zelle entstehen und in den extrazellulären Raum abgegeben werden. Sie haben einen Durchmesser von ca. 50-1000 nm und können, da sie im Blut zirkulieren, aus humanen Serumproben isoliert und mittels Durchflusszytometrie anhand ihrer Oberflächenproteine definiert und untersucht werden. Die Isolierung der taMP aus dem Serum von Patienten mit Kolorektalem Karzinom (CrC, n=55), Nicht-kleinzelligem Lungenkarzinom (NSCLC, n=43) und Pankreaskarzinom (PaCa, n=9) sowie von Patienten mit Struma (n=40) als negative Kontrolle und gesunden Kontrollpersonen (n=55) erfolgte mittels Zentrifugation, gefolgt von der Anfärbung der Mikropartikel mit tumorassoziierten Antikörpern. Auf diesem Wege konnte gezeigt werden, dass AnnexinV+EpCAM+ taMP und AnnexinV+EpCAM+CD147+ taMP im Serum der Tumorpatienten erhöht sind im Vergleich zu gesunden Probanden. Die diagnostische Leistung wird durch die AUROC-Werte unterstützt, die für die Messung der AnnexinV+EpCAM+ taMP von 0,7965 (NSCLC), über 0,8059 (CrC), bis 0,8675 (PaCa) reichen. Die Kombination der Antikörper EpCAM und CD147 kann zudem erfolgreich die Tumorpatienten von Patienten mit anderen Erkrankungen, in diesem Fall Struma, abgrenzen, und mit AUROC-Werten von 0,8672 (NSCLC), 0,8628 (CrC) und 0,9111 (PaCa) noch bessere diagnostische Akkuratesse vorweisen. Weiterführende Untersuchungen zeigen, dass AnnexinV+EpCAM+CD147+ taMP mit der Tumorlast der Patienten mit CrC korrelieren (r=0.7461) und dass die Anzahl der EpCAM+ taMP im Serum von Patienten mit CrC nach einer Resektion des Tumors absinkt (p=0,0052). Zusammenfassend lässt sich sagen, dass die Isolierung AnnexinV+EpCAM+ und AnnexinV+EpCAM+CD147+ taMP aus humanem Serum sich als neues, minimal invasives und kostengünstiges diagnostisches Mittel in der Vorsorge, Diagnose und eventuell der Therapieüberwachung von malignen Tumoren durchsetzen könnte.
Circulating tumor-associated microparticles: a new approach in diagnosis and therapy monitoring of the colorectal carcinoma and other epithelial neoplasia Cancer is one of the major causes of death in the world with approximately 8 million cancer-related deaths per year. Many cancer entities, such as non-small cell lung cancer or carcinomas of the pancreas, are often first diagnosed at a late stage. This is because the patients do not show typical symptoms in the early stages. However early diagnosis is crucial for successfull treatment and survival. Hence there is an urgent need for diagnostic tools that can detect cancer at an early stage. This study presents tumor-associated microparticles (taMP) as a new approach for early cancer diagnosis. Microparticles are cell derived extracellular vesicles that are formed by the outward budding of the cell membrane during stress, early apoptosis or Ca2+ increase in the cell. They are 50-1000 nm in diameter and released into the extracellular space. Therefore they can be isolated from human serum samples and characterised based on their surface proteins by flow cytometry. The isolation of the taMP from the serum of cancer patients with colorectal carcinomas (CrC, n=55), non-small cell lung cancer (NSCLC, n=43) and pancreatic carcinomas (PaCa, n=9), as well as patients with struma (n=40) as a negative control group and a control group of healthy persons (n=55) was effected by means of centrifugation followed by staining the microparticles with tumor-associated antibodies. With this method it is possible to show that AnnexinV+EpCAM+ taMP and AnnexinV+EpCAM+CD147+ taMP levels are increased in the serum of cancer patients compared to in that of healthy subjects. The diagnostic accuracy is supported by the AUROC-values. The values for AnnexinV+EpCAM+ taMP are 0.7965 for the patients with NSCLC, 0.8059 for those with CrC and 0.8675 for the group with PaCa. The combination of EpCAM and CD147 also successfully distinguishes patients with cancer from those with struma. This combination shows an even better diagnostic performance with values of 0.8672 (NSCLC), 0.8628 (CrC) and 0.9111 (PaCa). Further analyses show that AnnexinV+EpCAM+CD147+ taMP levels correlate with the tumor burdens of the patients with CrC (r=0.7461) and that the AnnexinV+EpCAM+ taMP level in the serum of patients with CrC decreases after tumor resection (p= 0.0052). In conclusion it can be said that the isolation of AnnexinV+EpCAM+ and AnnexinV+EpCAM+CD147+ taMP from human serum could prove to be a new, minimally invasive, cost effective diagnostic tool for screening, early cancer diagnosis and therapy monitoring.
