Abstract: | Im Rahmen des akuten Myokardinfarktes kommt es zu einer vaskulären und myokardialen
Inflammation, die zu tiefgreifenden strukturellen Veränderungen im Herzmuskelgewebe
führt, und als wesentlicher prognostischer Faktor angesehen wird. Diese
Inflammationsreaktion wird maßgeblich von Monozyten und Zytokinen wie IL-1α moduliert.
Das Ziel der durchgeführten Untersuchung war es, den Zusammenhang von
membrangebundener IL-1α Expression auf zirkulierenden Monozyten und Myokardinfarkten
zu untersuchen.
Hierzu wurden bei 58 Patienten, die mit der Diagnose NSTEMI oder STEMI in die
Intensivstation der Klinik für Innere Medizin III des Universitätsklinikums des Saarlandes
eingeliefert wurden, venöse Blutproben entnommen und Monozyten isoliert. Die frisch
isolierten Zellen wurden bezüglich der Expression von membrangebundenen IL-1α auf
klassischen CD14+ sowie auf CD14+CD16+ Monozyten durchflusszytometrisch untersucht.
Die Blutproben 15 gesunder Probanden dienten als Kontrolle.
Nach NSTEMI- und STEMI-Myokardinfarkt zeigte sich verglichen mit der gesunden
Kontrollgruppe eine signifikant höhere Expression von IL-1α auf klassischen CD14+ und
CD14+CD16+ Monozyten. Bei den CD14+ Monozyten von NSTEMI Patienten exprimierten
53,1% ± 28,3% und bei den CD14+ Monozyten von STEMI-Patienten 53,3% ± 32,6%
zusätzlich zu CD14 auch IL-1α auf ihren Oberflächen. Bei der Kontrollgruppe lag diese Wert
bei 14,1% ± 9,6% (p<0,0001, p<0,001). Bei den CD14+CD16+ Monozyten ergab sich ein
ähnliches Bild: In der Gruppe NSTEMI Patienten exprimierten 46,3% ± 29,6% der
Monozyten zusätzlich IL-1α und in der STEMI-Gruppe waren es 53,3% ± 25,2%. Auch hier
war der Unterschied zu den Kontrollproben, die eine Expressionsrate von 9,4% ± 8,7%
zeigten, signifikant (p<0,0001, p<0,0001).
Mit dieser Studie wurde gezeigt, dass IL-1α nach akutem Myokardinfarkt auf peripheren
Monozyten vermehrt exprimiert wird. Diese Daten legen nahe, dass IL-1α auf peripheren
Monozyten als proximaler Teil einer Signalkaskade angesehen werden kann, der die
Inflammation nach Herzinfarkt beeinflusst. The innate immune response following myocardial infarction triggers an intense inflammatory reaction, that can alter the topography of the ventricular structure and importantly affects the prognosis of survival. This inflammatory response is modulated by monocytes and cytokines such as IL-1α. In the present study, we sought to evaluate expression levels of IL-1α on peripheral blood monocytes in patients with ST-segment and non- ST-segment elevation acute myocardial infarction. Therefore, we obtained peripheral blood samples of 58 NSTEMI and STEMI patients, who were hospitalized in the cardiological intensive care unit of the Universitätsklinikum des Saarlandes, Homburg. The blood samples of 15 healthy persons served as normal controls. The expression of IL-1α on peripheral blood monocytes was determined by flow cytometry. Patients with NSTEMI and STEMI showed a significantly higher expression of membranebound IL-1α on classical CD14+ and on CD14+CD16+ monocytes, compared to healthy controls. While CD14+ and CD14+CD16+ monocytes in the NSTEMI group expressed membrane-bound IL-1α with an average of of 53,1% ± 28,3% or 46,3% ± 29,6% and CD14+ and CD14+CD16+ monocytes in the STEMI group demonstrated an average expression rate of 53,3% ± 32,6% or 53,3% ± 25,2%, IL-1α was expressed in only 14,1% ± 9,6% or 9,4% ± 8,7% of the healthy controls (p<0,0001, p<0,001). Our results indicate, that the expression of IL-1α on peripheral blood monocytes is upregulated after myocardial infarction. |
