Abstract: Zusammenfassung Ziel der Studie: Verschiedene Bestrahlungstechniken können für die Bestrahlung des Tumorbettes nach brusterhaltender Therapie bei Brustkrebs verwendet werden. Während der Brustbestrahlung erhält die ipsilaterale Axilla unvermeidlich eine gewisse Bestrahlungsdosis. Diese unbeabsichtigte Bestrahlung der axillären Lymphknoten könnte therapeutische Effekte haben. Ziel dieser Analyse war es, die Dosisverteilung im Bereich der axillären Lymphabflusswege bei zwei modernen Boost-Techniken nach einer intensitätsmodulierten Bestrahlung (IMRT) der Brust zu quantifizieren und anschließend zu vergleichen. Die Boost-Bestrahlung erfolgte entweder als simultan integrierter Boost (SIB) oder als sequenzieller Boost (SEQ). Ergänzend erfolgte eine Analyse der Dosisbelastung im Bereich der ipsilateralen Lunge und des Herzens. Material und Methoden: Es erfolgte eine monozentrische Analyse zweier Bestrahlungspläne mit jeweils unterschiedlicher Boost-Technik für Patientinnen mit Brustkrebs. 30 Patientinnen mit einem linksseitigen Mammakarzinom wurden retrospektiv ausgewählt. Für jeden Kasus lag der bereits klinisch verwendete Bestrahlungsplan der Brustregion mittels IMRT und mit einem simultan integrierten Boost vor. Für diese Analyse wurde ein zweiter Bestrahlungsplan mit einer IMRT der Brust und sequenziellem 3-Felder Boost erstellt. Die Brustdrüse wurde bei beiden Methoden mit einer Gesamtdosis von 50,4 Gy bestrahlt. Das Tumorbett erhielt bei der SIB-Methode 59,92 Gy und bei der SEQ-Methode 59,4 Gy. Die Dosisbelastung im Bereich der axillären Level I, II und III sowie der ipsilateralen Lunge und des Herzens wurden ermittelt. Die zwei unterschiedlichen Bestrahlungsmethoden wurden miteinander verglichen und statistisch mithilfe eines t-Test oder Wilcoxen-Test ausgewertet. Ergebnisse: Das Level I der Axilla erhielt bei der SIB-Methode im Mittel 38 Gy und bei der SEQ-Methode 39,78Gy. Diese Differenz war statistisch signifikant. Die Dosis im Level II und Level III lag bei beiden Methoden bei 18 Gy bzw. 11 Gy. Hier ergaben sich keine statistisch signifikanten Unterschiede zwischen beiden Methoden. 44 % des Volumens des Level I erhielt 45 Gy bei der SIB-Methode. Bei der SEQ-Methode waren es 48 %. Die untersuchten Variablen für die Dosisbelastung auf das Herz und die ipsilaterale Lunge waren bei der SIB-Methode durchschnittlich niedriger. Diese Differenzen waren für alle Variablen (Dmean Lunge, V20 Lun- ge, Dmean Herz) statistisch signifikant. Schlussfolgerung: Die berechneten Dosen im Bereich der axillären Level I, II und III lagen bei beiden Bestrahlungsmethoden unterhalb der üblicherweise therapeutisch verwendeten Bestrah- lungsdosen. Trotzdem erhielt etwa die Hälfte des Level I eine Dosis von 45 Gy, welche theoretisch eine therapeutische Auswirkung haben könnte. Die Risikoorgane, i. e. Herz und ipsilaterale Lunge, erhielten bei der Bestrahlung mittels simultan integriertem Boost geringere Bestrahlungsdosen.
Summary Introduction: Different radiation techniques can be applied in the treatment of the tumor bed after breast conserving therapy for breast cancer. During radiotherapy of the breast the ipsilat- eral axilla inevitably receives an amount of radiation. This incidental irradiation of the axillary lymph nodes could harbor a therapeutic effect. The goal of this analysis was to quantify and compare the axillary dose distributions between two modern boost techniques to the tumor bed following an intensity-modulated radiotherapy (IMRT) of the breast. The studied techniques consisted of a simultaneous integrated boost (SIB) and a sequential boost (SEQ). Additionally, the dose distribution in the ipsilateral lung and the heart were analyzed for both methods. Materials and Methods: A single institutional analysis of two radiation treatment plans with a different boost technique was carried out for patients with breast cancer. 30 patients with a left- sided tumor were retrospectively selected. The truly clinically applied radiation treatment plan for each patient, consisting of an intensity-modulated radiotherapy of the breast and a simultane- ously integrated boost to the tumor bed, was already available. A second treatment plan consist- ing of an IMRT of the breast and a 3-field sequential boost to the tumor bed was generated solely for use in this analysis. In both treatment plans the breast was irradiated with a total dose of 50,4 Gy. The tumor bed received a total dose of 59,92 Gy with the SIB technique and 59,4 Gy with the SEQ technique. The dose distribution in the axillary levels I, II and III as well as for the heart and the ipsilateral lung were determined for both radiation techniques. The radiation doses were then compared and their differences statistically analyzed with a paired t-test and a Wil- coxon signed-rank test. Results: Level I of the axilla received an average mean dose of 38 Gy with the SIB technique and 39,78 Gy with the SEQ technique. This dose difference was statistically significant. Level II and Level III received an average mean dose of 18 Gy, respectively 11 Gy. There was no statis- tically significant difference between the two radiation techniques. 44 % of the volume of Level I received 45 Gy with the SIB technique. With the SEQ technique this was 48 %. The evaluated doses for the heart and ipsilateral lung were lower with the SIB technique. These dose differ- ences were statistically significant for all variables (Dmean Lung, V20 Lung, Dmean Heart) Conclusion: The calculated radiation doses for Level I, II and III of the axilla with both of the analyzed techniques were lower than the commonly used therapeutic radiation doses. Nonethe- less approximately half of Level I of the axilla received 45 Gy, which theoretically could have a therapeutic impact. The organs at risk, i.e. Heart and ipsilateral Lung received lower radiation doses with the SIB technique.
