Abstract: Ziel der hier vorliegenden Arbeit ist es, anhand einer retrospektiven Datenanalyse den bisherigen Einsatz und die Wirksamkeit von Dronabinol bei Kindern in der Palliativmedizin und Kinderschmerztherapie der Universitätsklinik des Saarlandes im Hinblick auf die Symptome Übelkeit, Appetitlosigkeit, Schmerz, Spastik, neuropsychiatrische Symptome wie Angst, Depressionen und Schlafstörungen sowie Epilepsie zu untersuchen. Zur Analyse wurden die Patientenakten aller Patienten von Anfang 2010 bis einschließlich März 2018, bei denen Dronabinol als Therapieoption eingesetzt wurde, herangezogen, in eine Excel-Tabelle eingepflegt und mit SPSS ausgewertet. Alle stetigen Variablen wurden mittels Kolmogorov-Smirnov-Test auf Normalverteilung untersucht. Da es sich nicht um eine Normalverteilung handelt, wird zur Beschreibung der Ergebnisse Median mit Minimum und Maximum sowie der Bereich der 25. und 75. Perzentile angegeben. Bei der Analyse wurden insgesamt 124 Patienten (55 männliche und 69 weibliche Patienten) im Alter von drei Monaten bis 27 Jahren erfasst. Die Startdosierung lag täglich im Durchschnitt bei 0,1 mg/kg Dronabinol (2,5 mg), die Zieldosis lag täglich im Durchschnitt bei 0,21 mg/kg (6,0 mg). Die Kinder dieser Analyse profitierten durch Dronabinol zu 86 % bei Übelkeit, zu 76,6 % bei Appetitlosigkeit, zu 78 % bei Schmerzen, zu 85,5 % bei Spastik, zu 90 % bei neuropsychiatrischen Symptomen wie Angst, Depressionen und Schlafstörungen und zu 69,8 % bei Epilepsie. Schlussfolgernd lässt sich sagen, dass Dronabinol eine sinnvolle Therapieoption für Kinder in der Palliativmedizin und Kinderschmerztherapie darstellt. Zusammenfassung: Die positive Wirksamkeit von Dronabinol lässt sich bei den Symptomen Übelkeit, Erbrechen, Schmerz, Spastik, bei neuropsychiatrischen Symptomen wie Angst, Depression und Schlafstörungen und auch bei Epilepsie aufgrund der hier vorliegenden Datenerhebung belegen.
The aim of this study is to examine analytical research of the usage and effectiveness of dronabinol in children, treated in palliative care and paediatric pain therapy at the university hospital of Saarland regarding the symptoms nausea, anorexia, pain, spasticity, neuropsychiatric symptoms like anxiety, depression and insomnia and epilepsy. For the analysis we collected data from the beginning of 2010 until including march 2018 of all the medical records of patients treated with dronabinol for further therapeutic option and merged them into Excel and SPSS for the evaluation. All variables were testet for normal distribution using the Kolmogorov-Smirnov-Test. Because there was no normal distribution found, results are explained by using median with minimum and maximum and also the ranges based on the 25. and 75 percentiles. The analysis registered 124 patients in total (55 male and 69 female patients) at the age of 3 months up to 27 years. The average starting dosage of dronabinol per day was 0,1 mg/kg (2,5 mg), the average maximum dose of dronabinol per day was 0,21 mg/kg (6,0 mg). The children in the analysis had a benefit from dronabinol of 86 % regarding nausea, 76,6 % regarding anorexia, 78 % regaring pain, 85,5 % regarding spasticity, 90 % regarding neuropsychiatric symptoms like anxiety, depression and insomnia and 69,8 % regarding epilepsy. In conclusion, it can be said, that dronabinol is a useful therapeutic option for children in palliative care and paediatric pain therapy. Conclusion: The positive effects of dronabinol regarding nausea, anorexia, pain, spasticity, neuropsychiatric symptoms like anxiety, depression and insomnia and also epilepsy are substantiated by the available data.
