Abstract: | Die genaue Entstehung des Knochenmarködems ist nach wie vor nicht geklärt. Es
gibt verschiedene Theorien zur Genese und ob es überhaupt eine eigenständige
Erkrankung ist. In verschiedenen Studien hat sich gezeigt, dass es durch die Gabe
eines Prostaglandinanalogons (Iloprost) zu einer deutlichen Besserung der
Beschwerden bis hin zur kompletten Genesung kommt.
Darauf basierend wurde die Hypothese aufgestellt, dass das schmerzhafte
Knochenmarködem durch Mikroembolien im Knochenmark entsteht. Diese
Mikroembolien spiegeln einen gestörten Fettstoffwechsel und/ oder eine
Hypofibrinolyse wider. Eine Störung in diesen Systemen könnte durch veränderte
Serumwerte einfach gemessen werden. Daher könnten erhöhte Serumwerte der
Hypofibrinolyse und/ oder des gestörten Fettstoffwechsels sowohl einen Indikator als
auch einen Kontrollparameter für die Entstehung und den Verlauf des
Knochenmarködems darstellen. Die Behandlung mit dem stabilen
Prostaglandinanalogon Iloprost könnte dann zu einem Abfall oder sogar einer
Normalisierung der erhöhten Plasmawerte führen, welche auch mit einer
Schmerzreduktion einhergeht.
Im Rahmen einer prospektiven Kohortenstudie wurden 21 Patienten (12 Männer und
9 Frauen, mittleres Alter 50 Jahre (Range: 22-70 Jahre)) mit einem schmerzhaften
Knochenmarködem und erhöhten Lipoprotein (a) (Lp(a))- Werten im Serum mit
Iloprostinfusionen behandelt. Vor Beginn der Behandlung mit Iloprost und 6 Wochen
nach Beendigung der Therapie wurden die Serumkonzentrationen von Lp(a),
Apolipoprotein (A1) (Apo(A1)) und Apolipoprotein (B) (Apo(B)) bestimmt sowie eine
MRT-Verlaufskontrolle der betroffenen Region durchgeführt. In der Verlaufskontrolle
nach 6 Wochen berichteten 17 Patienten über eine komplette Beschwerdefreiheit.
Diese Patienten zeigten auch eine komplette Regredienz des Knochenmarködems
der zuvor betroffenen Region in der MRT-Kontrolle. Vor der Iloprost-Therapie waren
die durchschnittlichen Werte für Apo(A1), Apo(B) und Lp(a) 159,8, 108,3 bzw. 69,1
mg/dl. Sechs Wochen nach Iloprost-Therapie sanken die durchschnittlichen Werte für Apo(A1), Apo(B) und Lp(a) auf 147,6 (P=.011), 98,4 (P=.042) bzw. 38,3 (P<.001)
mg/dl. 4 Patienten berichteten über gleichbleibende oder progrediente Beschwerden.
Hier zeigte sich im MRT eine partielle Besserung der pathologischen Befunde. Die 4
Patienten hatten auch gleichbleibende oder weiter angestiegene Lp(a)-Werte. Somit
ergibt sich eine Erfolgsrate durch die Iloprost-Therapie von 86% mit einem
durchschnittlichen follow-up von 17 Monaten (Range 3-45 Monate).
Die Ergebnisse dieser Studie weisen auf eine mögliche Beteiligung der
Hypofibrinolyse und/ oder eine Störung des Fettstoffwechsels in der Entstehung des
schmerzhaften Knochenmarködems hin. Künftige Studien sollten die exakte
Zuordnung dieser Laborparameter klären und ob möglicherweise deren Bestimmung
als Ersatzmöglichkeit der Diagnostik gegenüber des aktuellen Gold-Standards (MRT)
dienen könnte. The exact emergence of the bone marrow edema is still not cleared. There are several hypotheses about the genesis and rather, if it is an own disease pattern. The treatment with a prostaglandin-analogue (iloprost) indicates an improvement of pain up to no discomfort which was shown in various studies. Based on this hypothesis the emergence of painful bone marrow edema might occur through microembolisms in the bone marrow. These microembolisms show a hypofibrinolysis and/ or a disturbance of the lipid metabolism. Changed plasma parameters show the disturbance in these systems and can be identified easily. Hence, an elevated plasma parameter may reflect the hypofibrinolysis and/ or the disturbance of the lipid metabolism, and act as both an indicator and a control parameter for the emergence and the process in bone marrow edema, respectively. A treatment with the stable prostaglandin-analogue iloprost may lead then to a decrease in or normalization of the elevated serum parameters and, therefore, to pain reduction. In the context of a prospective cohort study, 21 patients (12 men and 9 women, mean age 50 years (range: 22-70 years)) with a painful bone marrow edema and elevated lipoprotein (a) (Lp(a)) serum values were treated with intravenous iloprost. Before and 6 weeks after ending the iloprost therapy, the serum concentrations of Lp(a), apolipoprotein (A1) (Apo(A1)) und apolipoprotein (B) (Apo(B)) were determined as well as a MRI-follow-up-control of the affected region. At 6-week follow-up, 17 patients reported complete resolution of their symptoms. For these patients, complete bone marrow edema resolution was observed on magnetic resonance imaging of the affected region. Before iloprost therapy mean Apo(A1), Apo(B) and Lp(a) values were 159,8, 108,3 and 69,1 mg/dl respectively. Six weeks after iloprost therapy, mean Apo(A1), Apo(B) and Lp(a) values decreased to 147,6 (P=.011), 98,4 (P=.042) and 38,3 (P<.001) mg/dl, respectively. 4 patients reported of constant or progressive symptoms but had partial bone marrow edema resolution on magnetic resonance imaging. In these patients Lp(a) values either increased or remained the same. Hence, the total success rate of iloprost treatment was 86% at a mean follow-up of 17 months (range 3-45 months). The results of this study indicate a possible role of hypofibrinolysis and/ or a disturbance in the lipid metabolism in the emergence of painful bone marrow edema. Future studies should clear the definite correlation of this serum parameter. Maybe conduces the purpose of the serum parameter to standby facilities compared to the current gold-standard (MRI). |
