Title: Durchflusszytometrische Untersuchung endothelialer Mikropartikel pulmonal- und peripher-vaskulären Ursprungs bei PAH- und CTEPH-Patienten

Abstract: Hintergrund: Mikropartikel (MPs) sind Zellabschnürungen aus der Membran verschiedener Zelltypen mit einer Größe von unter einem 1μm. MPs sind entscheidend an der interzellulären Kommunikation beteiligt und gelten als biologische Marker für Prozesse wie Apoptose, Zellaktivierung und enodotheliale Dysfunktion. Die pulmonale Hypertonie (PH) ist eine Erkrankung, die mit endothelialer Dysfunktion speziell in der pulmonalen Strombahn einhergeht. Es wurde mehrfach gezeigt, dass unterschiedliche MP-Gruppen im Blut von PH-Patienten erhöht waren, die genaue pathophysiologische Rolle ist jedoch noch größtenteils ungeklärt. Methoden: 26 PH-Patienten (55,5±15,5 Jahre; PAPm 48,0±13,7mmHg), von denen 11 an einer PAH und 15 an einer CTEPH litten sowie 14 gesunde Kontrollprobanden wurden in diese Studie eingeschlossen. Einschlusskriterium für PH-Patienten und Probanden war, dass sie keine kardiovaskulären Erkrankungen hatten. Zum Zeitpunkt der Untersuchung standen 17 Patienten unter einer gezielten medikamentösen PH Therapie. Die MPs wurden mittels Durchflusszytometrie gemessen. Als Probenmaterial diente thrombozytenfreies Plasma (PFP) aus venösem Blut, das mittels CTAD antikoaguliert wurde. Um absolute Werte der MPs (counts/μl) zu messen, wurde zusätzlich zur Durchflusszytometrie noch eine Messung mittels CASY®-Zellzähltechnik durchgeführt. Die verschiedenen MP-Populationen wurden folgendermaßen festgelegt: Endotheliale MPs (EMPs): PECAM+ (CD31+/CD41–); aktivierte EMPs: E-Selectin+ (CD62+); thrombozytäre MPs (PMPs): GpIIb+ (CD41+); apoptotische MPs: Annexin V+ und pulmonale EMPs: ACE+ (CD143+). Ergebnissse: Die Konzentrationen endothelialer Mikropartikel (CD31+/CD41–) waren bei den PH-Patienten signifikant niedriger als bei den gesunden Kontrollprobanden (PH 116,32 ± 91,99 vs. Kontrolle 189,63 ± 138,69; p = 0,039). Innerhalb der PHSubgruppen fanden sich unterschiedliche MP-Konzentrationen. PAH-Patienten zeigten die höchsten Konzentrationen apoptotischer (AnnexinV+) EMPs. Diese Konzentrationen waren signifikant höher als die der CTEPH-Patienten (PAH 79,544 ± 78,33 vs. CTEPH 19,62 ± 40,13 ; p = 0,034). Dies galt sowohl für pulmonale (ACE+) (PAH 11,45 ± 8,95 vs. CTEPH 2,87 ± 3,48 p = 0,023) als auch für periphere (ACE–) EMPs (PAH 68,09 ± 72,86 vs. CTEPH 16,74 ± 37,07 ; p = 0,030). Keine signifikanten Konzentrationsunterschiede zeigten sich bei PH-Patienten und Kontrollen bezüglich der thrombozytären MPs (CD41+). Eine weitere endotheliale MP-Population mit einem starken ACE+ Signal im FACS, die als „bright“ bezeichnet wurde und vermutlich pulmonalendothelialen Ursprungs ist, zeigte eine positive Korrelation mit der rechtsventrikulären Arbeit (RVW= PAPm x RSV) (r = 0,631) und dem pulmonal vaskulären Widerstand (PVR) (r = 0,628). Patienten der WHO Funktionsklasse I und II zeigten ein signifikant höheres Verhältnis ACEbright EMPs zu schwächer ACEdim EMPs als Patienten in den WHO Funktionsklasse III und IV (WHO FC III-IV 1,489 ± 0,94 vs WHO FC I-II 0,713 ± 0,80 ; p = 0,011). Schlussfolgerung: Im Gegensatz zu vorausgegangenen Studien waren die EMPKonzentrationen (CD31+ ; CD41-) bei PH-Patienten in dieser Arbeit deutlich niedriger als bei gesunden Kontrollprobanden. Der Befund, dass Annexin V+ EMP Konzentrationen signifikant höher bei PAH als bei CTEPH waren, könnte darauf hinweisen, dass die endotheliale Dysfunktion in der Pathogenese der PAH eine wichtigere Rolle als bei CTEPH spielt. Die positive Korrelation von pulmonalen EMPs mit dem PVR und der rechtsventrikulären Arbeit sowie die Tatsache, dass sich die Spiegel pulmonaler EMPs bei verschiedenen WHO Funktionsklassen signifikant unterscheiden, zeigen, dass EMPs möglicherweise in der Zukunft als Marker zur Beurteilung des Schweregrades der Erkrankung dienen können. Noch ist die pathophysiologische Rolle der Mikropartikel unzureichend geklärt und auch die Nachweismethodik ist noch nicht ausreichend standardisiert, sodass keine endgültigen Schlüsse zur Rolle von MPs bei pulmonaler Hypertonie möglich sind.
Backround: Microparticles (MPs) are submicron membrane fragments (<1μm) that are shed by different types of cells. MPs play an important role in intercellular communication and are biological markers associated with apoptosis, cell activation and endothelial dysfunction. Pulmonary Hypertension (PH) is a disease associated with endothelial dysfunction especially in the pulmonary vascular bed. It has been reported that different subgroups of circulating MPs are elevetated in patients with PH, their pathophysiological role in the developement of PH still remains poorly understood. Methods: 26 Patients (55,5±15,5 age ; PAPm 48,0±13,7mmHg) with PAH (n=11) and CTEPH (n=15) without any other cardiovascular diseases as well as 14 healthy age- and sex-matched control subjects were included into the study. At the time most patients (n=17) received endothelial-active vasodilator therapy. Microparticles were measured by flow cytometry in platelet-free plasma from venous blood anticoagulated with CTAD. Additional samples were measured with CASY®- cell sorting technique to obtain absolute microparticle counts (counts/μl). Different MP-subgroups were characterized as followed: Endothelial microparticles (EMPs) were PECAM+ (CD31+/CD41–), activated EMPs E-Selectin+ (CD62E+), platelet derived MPs (PMPs) were GpIIb+ (CD41+) , pulmonary EMPs were ACE+ (CD143+) and apoptotic MPs were Annexin V+. Resulsts: Endothelial Microparticles (CD31+/CD41-) were significantly reduced in PHpatients compared with control-subjects (PH 116,32 ± 91,99 vs. Control 189,63 ± 138,69 ; p = 0,039). PH subgroups displayed significantly different MP levels. PAH patients showed significantly higher levels of Annexin V+ EMPs than CTEPH patients (PAH 79,54 ± 78,33 vs. CTEPH 19,62 ± 40,13 ; p = 0,034). This observation was made for both pulmonary (ACE+) EMPs (PAH 11,45 ± 8,95 vs. CTEPH 2,87 ± 3,48 p = 0,023) and for peripheral EMPs (ACE-) (PAH 68,09 ± 72,86 vs. CTEPH 16,74 ± 37,07 ; p = 0,030). No differences between levels of circulating PMPs (CD41+) were observed. Higher levels of a new EMP-population with a very strong ACE+ signal in FACS named bright correlated positively with right ventricular work (RVW = PAPm * SV) (r = 0,631) and pulmonar vascular resistance (PVR) (r = 0,628). Patients in WHO Functional Classes I and II showed significantly higher ratio of ACEbright EMPs compared with less strong ACEdim than patients in WHO Functional classes III and IV (WHO FC III-IV 1,489 ± 0,94 vs WHO FC I-II 0,713 ± 0,80 ; p = 0,011). Conclusions: In contrast to previous studies PH-patients showed lower levels of circulating EMPs (CD31+; CD41-). The fact that Annexin V+ EMP concentrations are significantly elevetated in PAH Patients compared to CTEPH patients suggests that endothelial dysfunction plays a more important role in the pathogenesis of PAH than in CTEPH. The positive correlation between pulmonary EMPs and haemodynamic parameters such as PVR and RV-Work and the fact that pulmonary EMP levels differ between different WHO functional classes point out that pulmonary EMPs could become a new tool to asses the severity of pulmonary Hypertension in the future. The pathophysiolgy of MPs in PH still remains partially understood. Because of this and the lack of standardization concerning the methodology it is not possible to make definitive comments on the the role of MPs in pulmonary hypertension.
