Title: Gentransfer zur Reparatur von Knorpelschäden durch therapeutische rekombinante adeno-assoziierte virale (rAAV)-Vektoren

Abstract: Fragestellung. Die Behandlung fokaler Knorpelschäden ist ein un-gelöstes Problem in Orthopädie und Unfallchirurgie. In der vorlie-genden Arbeit wurden an einem Modellsystem der Reparatur os-teochondraler Defekte in der Trochlea femoris von adulten Kanin-chen folgende Hypothesen untersucht: (1) Gentransfer von huma-nem Fibroblastenwachstumsfaktor 2 (FGF-2) durch rAAV-Vektoren verbessert die Zellproliferation im Reparaturgewebe von osteochondralen Defekten in vivo; (2) Gentransfer von humanem FGF-2 durch rAAV-Vektoren verbessert individuelle Parameter der Struktur des knorpligen Reparaturgewebes von osteochondralen Defekten in vivo; (3) Gentransfer von humanem FGF-2 durch rAAV-Vektoren verbessert die Reparatur des subchondralen Kno-chens von osteochondralen Defekten in vivo; (4) Gentransfer von humanem FGF-2 durch rAAV-Vektoren verbessert den strukturel-len Gesamtindex der osteochondralen Reparatur in vivo. Methoden. In die Trochlea femoris von 4 Kaninchen wurde eine zirkuläre osteochondrale Läsion von 3,2 mm Durchmesser ge-setzt. In die entstandenen Knorpeldefekte wurden 10 μl rAAV-Vektorlösungen mit Wachstumsfaktoren direkt appliziert. Bei je-dem Versuchstier wurde der Defekt auf einer Seite rAAV-hFGF-2 und kontralateral rAAV-lacZ (Kontrollgen) eingesetzt. Die Effekte der Anwendung eines rAAV-hFGF-2-Vektors zur Reparatur von osteochondralen Defekten im Kaninchenmodell wurden nach 4 Monaten in vivo studiert. Ergebnisse. Die Daten zeigen signifikante Verbesserungen nach rAAV-hFGF-2-Gentransfer für die Zellproliferation im Reparatur-gewebe, und die strukturellen Parameter „Füllung des Defektes“, „Zelluläre Morphologie“ und „Integration“ des kartilaginären Repa-raturgewebes. Hingegen fand sich kein signifikanter Unterschied bei der Reparatur des subchondralen Knochens. Das übergeord-nete Ergebnis dieser Studie ist die signifikante Verbesserung des strukturellen Gesamtindexes der osteochondralen Reparatur nach 4 Monaten in vivo durch rAAV-hFGF-2-Gentransfer. Schlussfolgerung. Ein rAAV-vermittelter hFGF-2-Gentransfer ver-bessert die strukturelle osteochondrale Reparatur nach 4 Monaten in vivo im Kaninchenmodell. Derartige molekulare Therapien könn-ten in der Zukunft zu verbesserten rekonstruktiven Verfahren für die Behandlung fokaler Knorpelschäden in Orthopädie und Unfallchirurgie führen.
Introduction. The treatment of focal articular cartilage lesions is an unsolved problem in orthopaedics and trauma surgery. In the pre-sent work, the following hypotheses were tested in a model sys-tem of repair of osteochondral defects in the trochlea femoris of adult rabbits: (1) gene transfer of human fibroblast growth factor-2 (FGF-2) improves cell proliferation in the repair tissue of oste-ochondral defects via rAAV-mediated gene transfer in vivo; (2) gene transfer of human FGF-2 improves individual parameters of the structure of cartilage repair tissue in osteochondral defects via rAAV vectors in vivo; (3) gene transfer of human FGF-2 improves the repair of the subchondral bone in osteochondral defects via rAAV vectors in vivo; (4) gene transfer of human FGF-2 improves the overall structural index of osteochondral repair via rAAV vec-tors in vivo. Methods. An osteochondral lesion of 3.2-mm diameter was creat-ed in the trochlea femoris of 4 rabbits. A 10-μl rAAV vector solu-tion carrying growth factor sequences was directly applied to the generated defects into the knee joints. Each animal received rAAV-hFGF-2 on one knee joint and rAAV-lacZ (control treatment) on the contralateral knee. The effect of applying the rAAV-hFGF-2 vector on the repair of osteochondral defects was examined in the rabit model after 4 months in vivo. Results. The results show significant improvements upon rAAV-hFGF-2 gene transfer in the cell proliferation indices in the carti-lage repair tissue and the stuctural parameters of "defect filling", "cell morphology", and "integration" of the repair tissue. However, there was no significant difference in the levels of subchondral bone repair. The critical result of the study is the significant im-provement of the structural overall index of osteochondral repair after 4 months in vivo mediated by rAAV-hFGF-2 gene transfer. Conclusion. RAAV-mediated hFGF-2 gene transfer improves the structural osteochondral repair after 4 months in vivo in a rabbit model of osteochondral defect. In the future, such a molecular therapy may help to enhance the reconstructive techniques for the treatment of focal cartilage damage in orthopaedics and trauma surgery.
