Abstract: | Die Dysphagie ist ein Symptom, welches zahlreiche Ursachen (entzündliche,
traumatische, neuro-, myogene, obstruktive und psychische Genese) haben kann.
Eine strukturierte Herangehensweise bei Erkrankungen beinhaltet eine ausführliche
Anamnese mit anschließender klinischer und apparativer Diagnostik. Letztere
umfasst in der Hals-Nasen-Ohrenheilkunde eine Spiegeluntersuchung, die
derzeitigen Goldstandards „Videofluoroskopie“ und „Fiberoptisch- endoskopische
Evaluation des Schluckens (FEES)“ sowie gegebenenfalls die Panendoskopie.
Ergänzend werden Computer- und Kernspintomographie eingesetzt.
Die Ultrasonographie, welche kein routinemässiges Verfahren in der Bewertung des
Schluckaktes darstellt, findet aktuell als nicht-invasive Untersuchungsmethode erste
Aufmerksamkeit in der Schluckdiagnostik, beschränkt sich bislang jedoch
hauptsächlich auf oropharnygeale Abschnitte und bedarf einer weiteren Evaluation,
welcher sich die vorliegende Arbeit widmet.
81 Patienten ohne eine erkennbare Schluckkrankheit wurden sonographisch
untersucht. Von diesen 81 Teilnehmern, deren Alter zwischen 19 und 66 Jahren lag,
waren 44 Probanden weiblich und 37 männlich. Das mittlere Alter lag bei
32,1 ± 13,8 Jahren. Mit einem BMI von durchschnittlich 25,4 ± 3,1 kg/m² waren die
männlichen Probanden knapp übergewichtig, die weiblichen mit einem solchen von
22,2 ± 3,2 kg/m² normalgewichtig.
Bei allen 81 Probanden konnten jeweils drei Schluckvideos im Bereich des oberen
Ösophagusdrittels mit unterschiedlichen Medien (Speichel, Wasser und
Götterspeise) angefertigt und ausgewertet werden.
Die sonographischen Bild- und Video-Aufnahmen wurden mit einem Siemens X300-
Ultraschallgerät vorgenommen. Die Beurteilung der Schluckvideos erfolgte mit dem Programm MAGIX VIDEO DELUXE, die statistische Datenauswertung mit dem
Statistik-Paket STATISTICA.
Die sonographische Darstellbarkeit von Zungengrund, intralaryngealer Region, Sinus
piriformis/Hypopharynx, Schilddrüsen und oberem Ösophagusdrittel wurde jeweils
hinsichtlich folgender Einflussgrössen analysiert: Alter (< 25 Jahre, ≥ 25 Jahre)
Geschlecht und BMI (normalgewichtig bis einschliesslich 25 kg/m², übergewichtig >
25 kg/m²).
Mit einer Treffsicherheit von über 90% gelang die Erfassung nahezu aller genannten
Strukturen, insbesondere des zervikalen Ösophagusabschnittes. Der mittlere
Durchmesser des Ösophagus belief sich in diesem Bereich auf 0,88 ± 0,10 cm. Die
Längenbestimmung des zervikal einsehbaren Abschnittes der Speiseröhre erbrachte
durchschnittliche Messungen von 5,78 ± 1,66 cm Länge. Höhere Messdaten im
Ösophaguslängsschnitt konnten bei jungen, schlanken und weiblichen Probanden
erzielt werden.
Als erschwert erwies sich die Einsehbarkeit des Sinus piriformis/Hypopharynx,
welcher sich in 39,5% der Fälle nicht darstellen liess. Ein statistisch signifikanter
Unterschied ergab sich für das Geschlecht (p < 0,022), das Alter (p < 0,001) und den
Body-Mass-Index (p < 0,001) mit der Schlussfolgerung, dass die Darstellung bei
weiblichen, jüngeren und normalgewichtigen Probanden mit grösserer Treffsicherheit
gelang.
Für den Leerschluck (Speichel; Schluckakt 1) wurde eine Schluckgeschwindigkeit
von 4,60 cm/s und damit die höchtste Schluckgeschwindigkeit aller Schluckakte
ermittelt. Die Durchschittsgeschwindigkeit des Spritzschlucks (Wasser; Schluckakt 2)
lag mit 2,90 cm/s hinter der des Götterspeiseschlucks (Schluckakt 3) mit 2,99 cm/s.
Extrapoliert man die errechneten Geschwindigkeiten auf die Gesamtlänge des
Ösophagus mit circa 25 cm, so lag die ösophageale Bolustransitzeit zwischen 5-8
Sekunden. Vergleicht man die mittleren Schluckgeschwindigkeiten der drei
Schluckakte untereinander, so finden sich statistisch signifikante Unterschiede zwischen Schluckakt 1 und 2, 1 und 3 (jeweils p < 0,001) sowie zwischen Schluckakt
2 und 3 (p = 0,040). Den langsamsten aller Schluckakte stellte der Spritzschluck dar.
Schlussfolgernd lässt sich sagen, dass die Ultrasonographie eine wertvolle
Untersuchungsmöglichkeit des Schluckaktes bedeutet. Sie kann wiederholt
ausgeführt werden, ist aufgrund fehlender Strahlenbelastung ein risikoarmes
Verfahren und erlaubt darüber hinaus insbesondere eine Beurteilung des oberen
Ösophagus und das Erkennen von Pathologien des Schluckaktes.
Weitere sonographische Studien zur Erarbeitung vergleichender Messdaten
zwischen Gesunden und Erkrankten müssen folgen. Dysphagia is a symptom which can have different causes: inflammatory, traumatic, neurogenic, myogenic, obstructive and psychological genesis. A structured approach to disease includes a detailed anamnesis with clinical and instrumental diagnostic. The otolaryngology uses mirror examination, the current gold standards "Videofluoroscopy" (VFSS) and "Fiberoptic-Endoscopic Evaluation of Swallowing (FEES)" as well as panendoscopy. Additionally computed tomography and magnetic resonance imaging can be used. Ultrasonography, which is not a routine procedure in the assessment of the swallowing act, is currently meant to be a new non-invasive examination method in swallowing diagnostics. Due to the fact that it has been mainly limited to oropharnygeal sections so far, the present study focusses on further evaluation. 81 patients without a recognizable swallowing disease were examined sonographically. Out of these 81 subjects, aged between 19 and 66 years, 44 subjects were female and 37 were male. The mean age was 32.1 ± 13.8 years. With an average BMI of 25.4 ± 3.1 kg/m², the male subjects were slightly overweight, whereas the females had a normal weight of 22.2 ± 3.2 kg/m². In all 81 subjects we were able to videotape and finally analyse three swallowing acts of the upper esophagus section using different boluses (saliva, liquid and jelly). The sonographic images and video recordings were taken with a Siemens X300 ultrasound device. For the evaluation of the swallowing videos we used the program MAGIX VIDEO DELUXE, the statistical data were analysed with the statistical package STATISTICA. The sonomorphological visualization of the involved swallowing anatomical structures like tongue, intralaryngeal region, sinus piriformis/hypopharynx, thyroid gland and upper esophageal section were examined for influences of age (< 25 years old, ≥ 25 years old), gender and BMI (normal weight up to and including 25 kg/m², overweight > 25 kg/m²). With an accuracy of more than 90%, the detection of almost all mentioned structures, especially the cervical esophageal segment, was achieved. The mean diameter of the esophagus in this part was 0.88 ± 0.10 cm. The average length of the visible portion of the cervical esophagus was 5.78 ± 1.66 cm. Higher measurements in the esophageal longitudinal section could be collected in young, slim and female subjects. The inspection of the sinus piriformis/hypopharynx, which could not be represented in 39.5% of cases, turned out to be difficult. A statistically significant difference was found for gender (p < 0.022), age (p < 0.001) and BMI (p < 0.001), with the conclusion of a more successful visualization in female, younger and normal weighted subjects. For the dry swallow (saliva, swallowing act 1) we determined a swallowing speed of 4.60 cm/s, which was the highest swallowing speed of all swallowing acts. The average speed of the wet swallow (liquid, swallowing act 2; 2.90 cm/s) was slower than the act of swollowing using jelly (swallowing act 3; 2.99 cm/s). By extrapolating the measurements of the swallowing speeds to the total length of the esophagus with approximately 25 cm, the total bolus transit time was between 5- 8 seconds. Compairing the average swallowing velocities of the three swallowing acts, statistically significant differences were found between swallowing act 1 and 2, 1 and 3 (p < 0.001 in both) and between swallowing act 2 and 3 (p = 0.040). The slowest of all swallowing acts was the wet swallow. In conclusion, it can be said that the ultrasonography is a valuable examination method of the swallowing act. It can be used repeatedly, is a low-risk procedure due to low radiation exposure, and furthermore allows the evaluation of the upper esophagus as well as the recognition of pathologies of the swallowing act. However, further sonographic studies for the development of comparative data between healthy persons and patients must follow. |
