Abstract: Die Therapie der skelettal bedingten Klasse-III-Dysgnathien erfordert eine kieferorthopädisch- kieferchirurgische Vorgehensweise. Hierbei spielt es keine Rolle, ob eine Mikrognathie des Oberkiefers oder eine Prognathie des Unterkiefers ursächlich ist. Obwohl dieser kombinierte Behandlungsansatz fast ausnahmslos erfolgreich ist und entsprechend in der Literatur gewürdigt wurde, gibt es nur relativ wenige Angaben zur Stabilität der Ergebnisse. Ziel der Untersuchung war es, die skelettale und dentoalveoläre Stabilität der Ergebnisse nach erfolgter Umstellungsosteotomie bei Patienten mit Angle-Klasse III zu beurteilen. Es wird geprüft, ob die Stabilität des Ergebnisses abhängig von der Ausprägung der Dysgnathie, der Art der erfolgten Operation (bignath/monognath), des Alters oder des Geschlechtes ist. Es wurden Unterlagen von 40 Patienten (21m, 19w) im Alter von 17 bis 47 Jahren ausgewertet. Alle Patienten wurden erfolgreich kieferorthopädisch-kieferchirurgisch behandelt. Die Patienten wurden in Abhängigkeit vom Operationstyp in zwei Gruppen eingeteilt: n=25 Patienten wurden bignath, n=15 Patienten monognath operiert. Zusätzlich wurden die gepoolten Daten aller n=40 Patienten analysiert. Kephalometrische Auswertungen fanden zu vier Zeitpunkten statt, nämlich vor Behandlung, vor Operation, nach Operation sowie nach Behandlungsabschluss. Hierbei wurden skelettale, dentale und Weichgewebeanalysen durchgeführt. Alle Daten wurden anhand bereits angefertigter Fernröntgenseitenbilder ausschließlich retrospektiv und anonymisiert erhoben. Abgesehen von signifikanten Änderungen, die in der Umstellungsosteotomie begründet sind – Vergrößerung von SNA und Verkleinerung von SNB – deuten die Ergebnisse auf Veränderungen hin, die dentoalveolär und im Weichgewebe hochsignifikant sind. Diese treten jedoch erst in der Phase nach kieferchirurgischem Eingriff bis zum Ende der Therapie auf. Postoperative Rezidive geschehen vornehmlich bei bignath operierten Patienten. Weitere Untersuchungen sollten Kofaktoren wie Wundheilungsstörungen oder intraoperative Komplikationen integrieren, da deren Einfluss andernfalls unerkannt bleibt.
The treatment of skeletal-related Class III malocclusions requires an orthodontic-maxillofacial surgical procedure. It does not matter if a micrognathic maxilla or mandibular prognathism is the cause. Although this combined treatment approach is almost always successful and was recognized accordingly in the literature, there are relatively few data on the stability of the results. The aim of this study was to evaluate the skeletal and dentoalveolar stability of the results after osteotomy in patients with Angle Class III. It is checked whether the stability of the results depends on the severity of the malocclusion, the type of surgical procedure (bignathic / monognathic), age or gender. The records of 40 patients (21m, 19w) in aged 17 to 47 years were evaluated. All patients received successful treatment including orthognathic surgery. The patients were divided depending on the type of operation in two groups: n = 25 patients received bignathic and n = 15 patients received monognathic surgery. In addition, the pooled data of all n = 40 patients were analyzed. Cephalometric evaluations took place four times, i.e. before treatment, before surgery, after surgery and after treatment completion. Skeletal, dental and soft tissue analyzes were performed. Data collection happened retrospectively and anonymously, using cephalograms taken during the course of treatment. Apart from significant changes that are explained by the type of osteotomy - enlargement of SNA and reduction of SNB - the results indicate that are highly significant dental and soft tissue changes. However, this only occurred during the phase after maxillofacial surgery until the end of therapy. Postoperative relapse occurs primarily in patients with bignathic surgery. Further studies should integrate cofactors such as wound healing or intraoperative complications, whose influence remains otherwise undetected.
