Abstract: Innerhalb von Sekunden bildet sich auf jeder dem Mundmilieu ausgesetzten Oberfläche eine Schicht aus Proteinen, Glykoproteinen, Kohlenhydraten und Lipiden, die sogenannte Pellikel. Sie ist der Mediator für Grenzflächeninteraktionen in der Mundhöhle. Die Pellikel zeigt ein ambivalentes Verhalten, indem sie sowohl protektive Eigenschaften besitzt, als auch Proteine, die als Rezeptoren die bakterielle Adhärenz und die Ausbildung eines strukturierten dreidimensionalen bakteriellen Biofilms begünstigen. Der Pellikel wird durch ihre Schlüsselrolle als Mediator zwischen Zahn und Mundmilieu eine besondere physiologische und pathophysiologische Bedeutung bei der Entstehung von Karies, Erosionen und Parodontopathien zugeschrieben. Die Pellikel gilt als möglicher Ansatzpunkt in der Bekämpfung von Karies, Erosionen und Parodontopathien. Die Erforschung der Bildung, Ultrastruktur, Zusammensetzung und Funktion der Pellikel stellen die Grundlage verschiedenster neuer präventiver Therapieansätze dar. Das Ziel dieser klinisch- experimentellen in- situ- Studie war es, das elektrophoretische Bandenmuster der in der Mundhöhle auf 10 verschiedenen Werkstoffen zu 3 unterschiedlichen Zeiten und an 2 unterschiedlichen Expositionsorten gebildeten Pellikel zu analysieren und den Nachweis von spezifischen Pellikelproteinen zu erbringen. Über einen Zeitraum von 1, 3 und 30 Minuten trugen 6 gesunde Probanden partielle Miniplastschienen im Unterkiefer. Auf den Miniplastschienen wurden zur Zungenseite (lingual) und zur Wangenseite (bukkal) jeweils 4 Prüfkörper des gleichen Werkstoffes montiert. 10 verschiedene Werkstoffe (Rinderschmelz, Goldlegierung, Keramik, Prothesenkunststoff, Teflon, Komposit, Glasionomerzement, Amalgam, Glas und Titan) wurden untersucht. Die Prüfkörper wurden poliert, gereinigt und desinfiziert. Nach der jeweiligen intraoralen Expositionszeit wurden die Miniplastschienen mit destilliertem Wasser abgespült und die Prüfkörper von der Schiene entfernt. Zur Pellikelgewinnung wurden die Prüfkörper mit SDS getränkten Schwämmchen abgewischt. Die Schwämmchen wurden mit Lämmli- Puffer versetzt, die Proteinlösung abzentrifugiert und denaturiert. Die Proteine wurden entsprechend ihrem Molekulargewicht im Polyacrylamid- Gel elektrophoretisch aufgetrennt, gefärbt und mit einer speziellen Software ausgewertet. In den nach verschiedenen Zeiten und Lokalisationen gepoolten Pellikelproben wurde nach spezifischen Pellikelproteinen (alpha- Amylase, Lysozym, Laktoferrin, Carboanhydrase I, Carboanhydrase II, Muzin I, sIgA, Glykosyltransferase B, Glykosyltransferase C, Glykosyltranferase D, prolinreiche Proteine) mittels Western- Blotting gesucht. Dazu wurden die im Polyacrylamid- Gel aufgetrennten Proteine elektrophoretisch auf eine Membran übertragen („geblottet“) und immunochemisch das jeweilige Zielprotein nachgewiesen. Die Proteinadsorption im Mundmilieu zeigte unabhängig vom Substrat, der Lokalisation, der Expositionszeit oder den Probanden eine hohe Uniformität. Bis zu 41 Banden im Molekulargewichtsbereich zwischen 2,9 kDa und 348,5 kDa konnten in der Pellikel auf den 10 untersuchten Werkstoffen gefunden werden. Die Mittelwerte der Molekulargewichte der Pellikelproteine auf allen Werkstoffen, an beiden Lokalisationen, bei allen Probanden und zu allen drei Zeiten der intraoralen Exposition zeigten ein sehr homogenes Verteilungsmuster. Bereits nach 1 Minute Bildungszeit wies die Pellikel ein ähnliches Proteinprofil wie nach 3- und 30 Minuten Bildungszeit auf, so dass die Pellikelbildung als ein schneller Prozess einzustufen ist. Der qualitative Nachweis der spezifischen Pellikelproteine alpha- Amylase, Lysozym, Laktoferrin, Carboanhydrase I, Carboanhydrase II, sIgA, Glykosyltransferase B, Glykosyltransferase C, Glykosyltrandferase D und der prolinreichne Proteine konnte in der initialen in- situ- Pellikel mittels Western- Blot erbracht werden. Die Pellikelbildung ist weitgehend unabhängig von der Substratoberfläche. Alle dem Mundmilieu und somit dem Speichel ausgesetzten Werkstoffe besitzen die Fähigkeit zur Ausbildung einer Pellikel. In Folgeuntersuchungen könnten nano- massenspektrometrische Analyseverfahren genutzt werden, um das Pellikelproteom auf verschiedenen Substraten im Detail aufzuklären.
On all solid surfaces exposed to the oral environment a layer composed of proteins, glycoproteins, carbohydrates and lipids, termed pellicle, is formed within seconds. The pellicle is characterized by ambivalent properties, since it provides protective properties as well as specific proteins that function as receptor molecules for the bacterial adherence and, thus, facilitate formation of a three- dimensionally structured bacterial biofilm. The pellicle acts as physiological mediator between tooth surface and oral environment and has been ascribed a key role in the development of caries, erosion and periodontitis. Modification of the pellicle layer has been supposed as a novel strategy to control and combat caries, erosion or periodontis. In- detail investigation of the pellicle`s formation, ultrastructure, composition and function, therefore, is essential for the development of new approaches in preventive dentistry. This in- situ study was conducted in order i) to analyse the electrophoretic protein profile of pellicles which were formed in the oral cavity on a variety of materials, at different sites and over differing periods of time and ii) to demonstrate the presence of specific pellicle proteins in the pellicle layer. Six healthy volunteers wore partial Miniplast splints in the lower jaw for 1, 3 and 30 minutes, respectively. On top oft the splints, 8 test specimens of identical materials were mounted (4 lingual and 4 buccal). Ten different materials were examined: bovine enamel, gold alloy, ceramic, prosthetic acrylic resin, Teflon, dental composite, glass ionomer cement, amalgam, glass and titanium. The test specimens were polished, cleaned and disinfected. After the respective exposure time, the Miniplast splints were rinsed with destilled water and the test specimens were removed from the splints. For pellicle removal, the test specimens were wiped with SDS- soaked sponges. Then, the sponges were exposed in Lämmli buffer and the protein solution was centrifuged and denaturated. The proteins were electrophoretically separated, stained and analysed with a special Software. In addition, pooled pellicle samples were examined for detection of the the following pellicle proteins via Western blotting: alpha-amylase, lysozyme, lactoferrin, carbonic anhydrase I, carbonic anhydrase II, mucin I, sIgA, glycosyltransferase B, glycosyltransferase C, glycosyltranferase D, proline- rich proteins. Results: The protein adsorption in the oral environment was neither affected by the solid substrate material, nor by the sites and times of intraoral exposure, or subject. Up to 41 bands between 2,9 kDa and 348,5 kDa were found in the electrophoretical profile of the protein solution from the pellicles of the 10 examined materials. The mean molecular weight of the pellicle proteins was very similar among materials, oral sites, exposure times and subjects. After being exposed to the oral environment for 1 minute, the pellicle`s protein profile was already very similar to those which were detected after 3 and 30 minutes of the intraoral exposure. This indicates that the pellicle formation occurs very rapidly. The qualitative proof for the specific pellicle proteins alpha-amylase, lysozyme, lactoferrin, carbonic anhydrase I, carbonic anhydrase II, sIgA, glycosyltransferase B, glycosyltransferase C, glycosyltranferase D, and proline- rich proteins was revealed by Western blotting. The results of this study indicate that the formation of pellicles is not distinctly affected by the material surface. Pellicles will develop on all materials which are exposed to the oral environment. Future studies might use nano mass spectometric analysis methods in order to clarify in more detail the pellicle`s proteome on different substrates.
