Abstract: | Die Reduktion der Applikationsdosis ist eine einfache und wirkungsvolle Strahlenschutzmaßnahme, um die
schädliche Wirkung der im Rahmen der medizinischen Diagnostik eingesetzten, ionisierenden Strahlung zu
reduzieren. Ziel der Arbeit war den Einfluss von zwei unterschiedlichen Bildrekonstruktionsalgorithmen in
der Computertomographie - zum einen die herkömmliche gefilterte Rückprojektion und zum anderen die
neue iterative Bildrekonstruktion, die unmittelbar mit der Applikationsdosis in Zusammenhang stehen - auf
die Strahlenbelastung der Patienten und die Bildqualität mittels einer Phantomstudie und einer retrospektiven
Patientenstudie zu untersuchen.
In der in-vitro-Studie wurde zunächst der Einfluss des Röhrenstroms, der Röhrenspannung und des
Bildrekonstruktionsverfahrens auf die Bildqualität evaluiert, wobei objektive Bildqualitätsparameter wie das
Signal-Rausch-Verhältnis, das Kontrast-Rausch-Verhältnis und die Kantenschärfe mit Hilfe eines
Gefäßmodells analysiert wurden. Hierbei zeigte sich, dass die höchsten Werte des Signal-Rausch-
Verhältnisses und des Kontrast-Rausch-Verhältnisses bei einer Röhrenspannung von 80 kV, einem
Röhrenstrom von 400 mAs und iDose4 Level 6 ermittelt wurden. Allerdings führte die Erhöhung des
Röhrenstroms von 30 mAs auf 200 mAs zu einer deutlicheren (über 50%) Steigerung des Signal-Rausch-
Verhältnisses und des Kontrast-Rausch-Verhältnisses als die Erhöhung von 200 mAs auf 400 mAs (bis
12%). Die Auswertung der Kantenschärfe zeigte die höchsten Werte bei einer Röhrenspannung von 80 kV;
die Einstellungen des Röhrenstroms sowie des Bildrekonstruktionsverfahrens beeinflussten die Resultate
nicht. Hinsichtlich des Bildrauschens wurde eine deutliche (z. T. über 30%) Reduktion nach der Anwendung
der iterativen Bildrekonstruktionen erreicht.
In Anbetracht der Ergebnisse dieser Phantom-Studie könnten perspektivisch Protokolle mit einer reduzierten
Röhrenspannung (z.B. 80 kV) in Kombination mit höheren Stufen von iDose4 (z.B. Level 4-6) für die
Volumenreduktion des intravenösen Kontrastmittels evaluiert werden. Aufgrund einer hohen
Strahlungsdurchlässigkeit des Lungenparenchyms wäre diese Anwendung für die hochkontrastierte
Gefäßdarstellung im Thoraxraum (z.B. der Aorta oder der Pulmonalarterien) besonders interessant.
In der retrospektiven in-vivo-Studie, bei der ein intraindividueller Vergleich von computertomographischen
Aufnahmen mit gefilterter Rückprojektion und iterativer Bildrekonstruktion durchgeführt wurde, sollte zum
einen der Einfluss der Bildrekonstruktionstechnik auf die Strahlenbelastung der Patienten untersucht und
zum anderen die in der in-vitro-Studie erhobenen theoretischen Erkenntnisse überprüft werden. In diesem
Teil der Studie konnte gezeigt werden, dass in der klinischen Routine durch die Verwendung der iterativen
Bildrekonstruktion (iDose4 Level 2 bzw. 3) die applizierte Strahlendosis bis zu 61% reduziert wurde. Die
Analyse der objektiven Bildqualitätsparameter haben ergeben, dass es nach Einführung der iterativen
Bildrekonstruktion zu einer leichten Erniedrigung des Signal-Rausch-Verhältnisses und des Kontrast-
Rausch-Verhältnisses gekommen ist, was allerdings in erster Linie auf die veränderten
Untersuchungsprotokolle mit deutlich geringeren Kontrastmittelvolumina (ca. 30%) zurückzuführen ist.
Nach den Erfahrungen aus der in-vitro-Studie zufolge, wären ohne die iterative Bildrekonstruktion noch niedrigere Signal- und Kontrast-Rausch-Verhältnissen zu erwarten gewesen. Die leicht erniedrigten Signalund
Kontrast-Rausch-Verhältnisse hatten jedoch keinen Einfluss auf die diagnostische Aussagekraft der
Untersuchungen. Die subjektive Analyse der Bilder durch zwei Untersucher ergab, dass die iterative
Bildrekonstruktion trotz der deutlich geringeren Strahlenbelastung gleich hochwertige
Untersuchungsergebnisse liefert wie die gefilterte Rückprojektion mit deutlich höherer Strahlenbelastung.
In Zukunft könnte möglicherweise durch eine weitere Erhöhung der Rekonstruktionsstufe (z.B. iDose4 Level
4 bzw. 5) die Bildqualität bei gleich hoher Strahlenbelastung erhöht werden oder bei gleichbleibender
Bildqualität die Strahlenbelastung der Patienten weiter reduziert werden. The reduction of application dose is a simple and effective method to reduce the harmful effects of the ionizing radiation in the medical diagnostics. The aim of the study was to investigate the influence of two different image reconstruction algorithms in computer tomography (firstly the standard filtered back projection and secondly, the new iterative image reconstruction) on the radiation exposure of patients and the image quality by means of a phantom study and a retrospective patient study. Initially, in the in-vitro study the influence of the tube current, the tube voltage and the image reconstruction method of the image quality was evaluated, whereby objective image quality parameters such as signal-tonoise ratio, the contrast-to-noise ratio and edge sharpness were analyzed using a vessel model. The highest values of the signal-to-noise ratio and the contrast-to-noise ratio were determined at a tube voltage of 80 kV, a tube current of 400 mAs and iDose4 Level 6. However, the increase of the tube current of 30 mAs to 200 mAs led to a more pronounced increase (over 50%) of the signal-to-noise ratio and the contrast-to-noise ratio than the increase of 200 mAs to 400 mAs (up to 12%). The evaluation of the edge sharpness showed the highest values at a tube voltage of 80 kV; the settings of the tube current and the image reconstruction method did not influence results. The image noise achieved a significant reduction (partly over 30%) by using iterative image reconstruction. Considering the results of this phantom study, protocols could be modified in future with a reduced tube voltage (for example, 80 kV) in combination with higher levels of iDose4 (example Level 4-6) for volume reduction of intravenous contrast agent. Due to an increased radiation permeability of the lung this application might be especially interesting for the highly contrasted angiography in the thorax (for example, the aorta or of the pulmonary arteries). In the retrospective in-vivo study, in which an intra-individual comparison of computed tomography scans with filtered back-projection and iterative image reconstruction were performed, the influence of image reconstruction technique on the radiation exposure were investigated. This part of the study showed that in the clinical routine, the applied radiation dose could be reduced to up to 61% by using iterative image reconstruction (iDose4 Level 2 or 3). The analysis of the objective picture quality parameters has shown that it has come to a slight reduction in the signal-to-noise ratio and the contrast-to-noise ratio after the introduction of iterative image reconstruction, which, however, primarily was caused by the changed examination protocols with significantly lower contrast medium volumes (ca . 30%). Based on the findings in the in vitro study, even lower signal and contrast-to-noise ratios would have been expected without the iterative image reconstruction. However, the slightly reduced signal and contrast-to-noise ratio had no effect on the diagnostic significance of the studies. The subjective analysis of images by two examiners showed that the iterative image reconstruction, despite the significantly lower radiation dose provides equally high quality study results as the filtered back projection with significantly higher radiation exposure. In future, the image quality might be increased with the same radiation exposure by further increase of the reconstruction level (as iDose4 Level 4 or 5) or the same image quality might be obtained with lower radiation exposure. |
