Abstract: | Bisphosphonate werden zur Therapie der Osteoporose und in der Onkologie bei ossärer
Metastasierung eingesetzt. Die Zoledronsäure ist das am häufigsten verordnete
Bisphosphonat bei Mammakarzinom-Patientinnen sowohl mit als auch ohne ossäre
Metastasen. Obwohl Bisphosphonate oft verordnet werden, sind das Spektrum und die
Häufigkeit der unerwünschten Arzneimittelwirkungen insbesondere bei Mammakarzinom-
Patientinnen noch nicht ausreichend untersucht. Insbesondere Daten zur Kieferosteonekrose
im adjuvanten und palliativen Einsatz von Zoledronsäure sind spärlich. Des Weiteren ist
noch ungeklärt, inwiefern die Ausprägung von Nebenwirkungen durch Bisphosphonate von
klinischen und therapeutischen Faktoren abhängen. Ziel dieser Studie war es, diese
Fragestellungen anhand der Daten von Mammakarzinom-Patientinnen, die Zoledronsäure
erhalten haben, zu klären. Ein weiteres Anliegen war es, die Compliance der Patientinnen in
Bezug auf die empfohlene zahnärztliche Kontrolle des Zahnstatus vor Therapiebeginn, die
empfohlene Durchführung einer Knochendichtemessung und die Regelmäßigkeit der
Infusionsgabe zu überprüfen.
In diese retrospektive Studie wurden alle Mammakarzinom-Patientinnen mit und ohne ossäre
Metastasen eingeschlossen, welche im Zeitraum 2009 bis 2010 an der Klinik für
Frauenheilkunde, Geburtshilfe und Reproduktionsmedizin am Universitätsklinikum des
Saarlandes Zoledronsäure erhalten haben. Die Datenerhebung erfolgte über digitalisierte
Patientinnenakten und Fragebögen, welche den Patientinnen per Post zugesandt wurden.
Insgesamt konnten Daten von n=79 Mammakarzinom-Patientinnen ausgewertet werden, die
im genannten Zeitraum Zoledronsäure adjuvant oder palliativ erhalten haben. 38,0% der
Patientinnen gaben unerwünschte Arzneimittelwirkungen an, wohingegen 26,6% keine
unerwünschten Arzneimittelwirkungen angaben. Patientinnen mit bzw. ohne unerwünschte
Arzneimittelwirkungen unterscheiden sich signifikant bzgl. des Tumorstadiums (p=0,024) und
des Nodalstatus (p=0,017) bei Erstdiagnose. Patientinnen mit ossären Metastasen und
daher 4-wöchentlichem Applikationszyklus von Zoledronsäure gaben genauso häufig
unerwünschte Arzneimittelwirkungen wie keine unerwünschten Arzneimittelwirkungen an
(jeweils n=6 Patientinnen). Adjuvante Patientinnen mit 6-monatlichem Applikationszyklus
gaben häufiger unerwünschte Arzneimittelwirkungen (n=18) als keine unerwünschten Arzneimittelwirkungen (n=13) an, der Zusammenhang von unerwünschten
Arzneimittelwirkungen und dem Applikationszyklus ist jedoch nicht signifikant (p=0,799). Als
häufigste unerwünschte Arzneimittelwirkungen wurden Knochenschmerzen (n=9),
Beschwerden des Magen-Darm-Trakts (n=6), grippale Infekte (n=6) und Fieber (n=5)
genannt. In n=4 Fällen kam es zur bisphosphonatassoziierten Kieferosteonekrose. Die
Patientinnen mit Kieferosteonekrosen bekamen Zoledronsäure in n=2 Fällen in
4-wöchentlichem, in n=1 Fall 3-monatlichem und in n=1 Fall 6-monatlichem Intervall. In
Bezug auf die Patientinnen-Compliance haben 50% der Patientinnen die empfohlene
zahnärztliche Kontrolle des Zahnstatus vor Therapiebeginn durchführen lassen. Die
empfohlene Knochendichtemessung wurde bei doppelt so vielen Patientinnen durchgeführt
wie nicht durchgeführt (n=34 vs. n=17). 86,1% aller Patientinnen haben die Zoledronsäure-
Infusion regelmäßig erhalten.
Die Studie zeigt, dass die Häufigkeit der unerwünschten Arzneimittelwirkungen von
Zoledronsäure bei Mammakarzinom-Patientinnen signifikant vom Tumorstadium und dem
Lymphknotenstatus abhängt. Ein signifikanter Zusammenhang zwischen dem
Vorhandensein von unerwünschten Arzneimittelwirkungen und dem Applikationszyklus
konnte nicht nachgewiesen werden. Das Auftreten von Kieferosteonekrosen ist mit 5,1%
selten und zeigt keinen Zusammenhang mit dem Applikationszyklus. Die Patientinnen-
Compliance ist mit 50% am niedrigsten in Bezug auf die empfohlene zahnärztliche
Untersuchung, liegt höher bezüglich der Knochendichtemessung und ist mit 86,1% sehr
hoch in Bezug auf die Regelmäßigkeit der Infusionsgaben. Bisphosphonates are used to treat osteoporosis and osseous metastatic spread in oncology. Zoledronic acid is the most commonly prescribed bisphosphonate in breast cancer patients with and without osseous metastases. Although bisphosphonates are often prescribed, the spectrum and the frequency of side effects particularly in breast cancer patients are still not sufficiently evaluated. In particular, data for osteonecrosis of the jaw in adjuvant and palliative use of zoledronic acid is sparse. Furthermore, it is still unclear how far clinical and therapeutic factors affect the manifestation of side effects in therapy with bisphosphonates. The aim of this study was to clarify these questions based on the collected data from breast cancer patients who received zoledronic acid. Another concern was the examination of the compliance of these patients regarding the recommended medical evaluation of the dental status before the beginning of the treatment, the recommended implementation of a bone densitometry and the regularity of the infusions. This retrospective study includes all patients with breast cancer with or without osseous metastases, that received zoledronic acid in the period from 2009 to 2010 at the Clinic of Gynaecology, Obstetrics and Reproductive Medicine at the Saarland university hospital. Data was collected via digitized patient files and questionnaires, which were sent to the patients by mail. Overall, data was evaluated from n=79 breast cancer patients who received zoledronic acid in the mentioned period as adjuvant or palliative therapy. 38.0% of the patients reported side effects whereas 26.6% did not report any side effects. Patients with or without side effects differ significantly in relation to tumor stage (p=0.024) and nodal status (p=0.017) at initial diagnosis. Half of the patients with bone metastases who therefore had an application cycle of four weeks of zoledronic acid, reported side effects. The other half didn´t suffer from any (n=6 patients each). Patients receiving adjuvant therapy in form of a six month cycle, indicated side effects more frequently (n=18) than no side effects (n=13). The relation between side effects and the application cycle is not significant (p=0.799). The most frequent side effects were bone pain (n=9), gastrointestinal symptoms (n=6), common cold (n=6) and fever (n=5). In n=4 cases (5.1%) bisphosphonate associated osteonecrosis of the jaw occurred. Patients with osteonecrosis of the jaw received zoledronic acid in n=2 cases every four weeks, in n=1 case every three months and in n=1 case every six months. Regarding patients’ compliance, half of the patients had passed the recommended medical evaluation of dental status before the beginning of the treatment. Twice as many patients had passed the recommended bone densitometry as patients that had not (n=34 vs. n=17). 86.1% of all patients had received zoledronic acid infusions regularly. The study shows that the incidence of side effects from zoledronic acid in breast cancer patients significantly depends on the tumor stage and nodal status. A significant correlation between the presence of side effects and the application cycle has not been proven. The occurrence of osteonecrosis of the jaw is rare with 5,1% and shows no correlation to the application cycle. The patients’ compliance is lowest regarding the recommended dental examination (50,0%), is higher regarding the bone densitometry and is very high regarding the regularity of the infusions (86,1%). |
