Abstract: Bei kardiologischen Patienten treten häufig psychische Komorbiditäten wie Angst und Depression auf. Diese Komorbiditäten können die Entstehung, die Behandlung und den Verlauf von Herzkrankheiten ungünstig beeinflussen. Die Lebensqualität (LQ) ist bei Menschen mit kardialen Erkrankungen zum einen durch ihre körperliche Erkrankung, zum anderen durch psychische Belastungen oftmals eingeschränkt. Aufgrund der großen Anzahl von Menschen, die von kardiovaskulären Erkrankungen betroffen sind, ist die Untersuchung der Verbindung zwischen Herz und Psyche bedeutsam. Gegenstand dieser Dissertation ist die Interaktion von Herz und Psyche mit einem Schwerpunkt auf Herzangst vor und nach Einsetzung eines implantierbaren Cardioverter Defibrillators (ICD). Nach einer theroretischen Einführung in das Themengebiet wird deshalb zunächst in einer Überblicksarbeit der Zusammenhang psychischer und kardiologischer Parameter dargestellt. Die Furcht vor kardialen Stimuli gepaart mit erhöhter Selbstaufmerksamkeit und Vermeidungsverhalten wird als Herzangst bezeichnet. Herzangst sollte frühzeitig erkannt werden, um intervenieren zu können und negative Auswirkungen auf den Krankheitsverlauf und die LQ sowie ökonomische Belastungen für das Gesundheitssystem zu mindern. Um das Risiko eines plötzlichen Herztodes bei Patienten mit Herzinsuffizienz zu minimieren, wird oftmals ein ICD eingesetzt. Dies kann einserseits ein Sicherheitsgefühl vermitteln, andererseits jedoch Angst vor Elektroschocks durch das Gerät bedingen. Um den Effekt eines ICD auf die Psyche und insbesondere die Herzangst untersuchen zu können, muss zunächst die psychische Verfassung vor Implantation analysiert werden. Deshalb werden in dieser Arbeit die Ausprägung und die Prädiktoren der Herzangst vor Implantation untersucht. Fraglich ist, ob die Implantation die psychische Verfassung beeinflusst und ob unterschiedliche Arten von Defibrillatoren verschiedene Auswirkungen auf psychische Faktoren haben. In einer weiteren Studie wird deshalb der Verlauf von Herzangst, Depressivität, Angst und LQ im Verlauf von fünf Monaten nach Implantation erhoben und auf Unterschiede zwischen Patienten mit ICD oder kardialer Resynchronisationstherapie mit Defibrillator (CRT-D) hin untersucht. Die Ergebnisse der Studien legen nahe, dass Herzangst ein von Angst zu unterscheidendes eigenständiges Konstrukt ist. Bei Patienten mit Herzinsuffizienz und Indikation zur ICD-Implantation ist die Herzangst stark ausgeprägt und wird durch niedrige LQ, allgemeine Angst und stattgehabten Myokardinfarkt vorhergesagt. Bei CRT-D-Trägern sinken herzbezogene Ängste nach Defibrillator-Implantation und die LQ verbessert sich im Vergleich zu den ICD-Trägern. Die Studien zeigen, dass Herzangst und weitere psychische Belastungen bei Patienten mit Herzinsuffizienz und ICD-Indikation stark ausgeprägt sind. Die psychische Symptomatik verschlimmert sich jedoch nicht nach Implantation eines Defibrillators, sondern verbessert sich bei Patienten mit CRT-D. Kardiologische Patienten, insbesondere Herzinsuffiziente vor und nach ICD-Implantation, sollten regelmäßig auf psychische Belastungen hin gescreent werden. Indem man belasteten Patienten frühzeitig zu einer adäquaten Beratung und gegebenenfalls einer psychotherapeutischen Intervention verhilft, kann einer Chronifizierung von psychischen Belastungen und Störungen vorgebeugt werden.
Psychological comorbidities like depression and anxiety frequently occur in cardiac patients. These comorbidities can have adverse effects on development, treatment and progression of heart diseases. Quality of life (QoL) is often impaired in patients with cardiac disease due to their physical illness and psychological distress. Owing to the high number of people affected by cardiovascular disease, the investigation of the link between heart and psyche is relevant. The objective of this dissertation is to analyse the interaction of heart and psyche with an emphasis on heart-focused anxiety (HFA) before and after placement of an implantable cardioverter defibrillator (ICD). After a theoretical introduction to the topic, a review of the correlation between psychological and cardiological parameters is presented. The fear of cardiac stimuli paired with enhanced self-attention and avoidance behaviour is termed as HFA. HFA should be identified at an early stage to successfully intervene and to reduce negative consequences for the course of the disease, for QoL and for the economic burden of the health care system. In patients with heart failure an ICD is oftentimes used to minimise the risk of sudden cardiac death. On the one hand, this may give a feeling of increased security but on the other hand it may induce fear of electric shocks emitted by the device. To study the effect of an ICD on the psyche and especially on HFA it is necessary to analyse the psychological condition prior to implantation. Thus, the manifestation of HFA and its predictors before implantation are investigated in this dissertation. It is questionable whether the implantation affects the psychological state and whether different types of defibrillators affect psychological factors diversely. Therefore, in another study the course of HFA, depression, general anxiety and QoL are measured five months after implantation and tested for differences between patients with ICD or cardiac resynchronisation therapy with defibrillator (CRT-D). The study results suggest that HFA is a construct which is distinct from general anxiety. HFA is highly pronounced in patients with heart failure and indication for ICD-implantation and is predicted by low QoL, general anxiety and former myocardial infarction. After implantation of an defibrillator heart-related fears decrease and QoL increases in patients with CRT-D compared to patients with ICD. The studies evidence that HFA and further psychological distress are highly present in patients with heart failure and ICD indication. However, the psychological symptoms are not aggravated by the implantation of a defibrillator but rather improve in patients with CRT-D. Cardiology patients, especially heart failure patients before and after ICD implantation, should be screened for psychological distress on a regular basis. The chronification of psychological strains and mental disorders may be prevented by providing the patient with adequate counseling and, if applicable, psychotherapeutic interventions at an early stage.
