Abstract: 1.1 Zusammenfassung Einleitung Normoblasten (engl. Nucleated red blood cells) sind die Vorl auferzellen von Erythrozyten und kommen beim Gesunden nur im Knochenmark vor. Bei Fr uh- und Neugeborenen sind sie jedoch physiologischerweise auch im peripheren Blut zu nden. Erh ohte Werte sind u.a. ein Indikator f ur intrauterinen Sauersto mangel. Ein weiterer prognoserelevanter Biomarker in der Neonatologie ist die Serumlaktat-Konzentration, die zur Beurteilung einer aeroben oder anaeroben Sto wechsellage dient. Eine Assoziation erh ohter Werte mit einem ung unstigen neonatalen Outcome wird diskutiert. Fragestellung Ziel unserer Arbeit war es, Referenzwerte f ur Normoblasten- und Serumlaktat-Konzentrationen f ur Very low birth weight (<1500 Gramm) - und Extremely low birth weight (<1000 Gramm) - Neugeborene sowie f ur Neonaten mit intrauteriner Wachstumsretardierung in den ersten 6 Lebenstagen zu erstellen und eine Assoziation zwischen den beiden Blutparametern zu ermitteln. Zus atzlich wurde die Bedeutung der beiden Blutparameter f ur das neonatale Outcome untersucht. Als letztes wurde die Rolle der Normoblasten- und der Serumlaktat-Konzentration im Kontext mit weiteren neonatalen prognoserelevanten Parametern analysiert. Material und Methodik Bei dieser Studie handelt es sich um eine retrospektive Analyse, die auf der neonatologischen Intensivstation des Universit atsklinikums des Saarlandes, Homburg/Saar, durchgef uhrt wurde. Es wurden insgesamt 250 Neugeborene zwischen 01/2009 und 12/2012 mit einem Geburtsgewicht <1500 Gramm in die Studie eingeschlossen und hinsichtlich ihrer Normoblasten- und Serumlaktat-Konzentration in den ersten 6 Lebenstagen sowie ihrem neonatalen Outcome untersucht. Ergebnisse Unser Patientenkollektiv umfasste 250 Patienten (123 (49,2%) m annlichen und 127 (50,8%) weiblichen Geschlechts) mit 140 (56%) Very low birth weight- und 110 (44%) Extremely low birth weight-Neugeborenen (mittleres Geburtsgewicht: 1048 g 305 g; Spanne: 320-1499 g). Die Normoblasten-Konzentration korrelierte hoch signi kant mit der Serumlaktat-Konzentration. Au erdem bestand eine inverse Korrelation beider Blutparameter mit dem Geburtsgewicht und dem Gestationsalter. Neonaten mit intrauteriner Wachstumsretardierung hatten signi kant h ohere Normoblasten- und Serumlaktat- Konzentrationen. Auf diesen Ergebnissen aufbauend wurden Normwerte f ur verschiedene Gewichtsklassen und Neonaten mit intrauteriner Wachstumsretardierung erstellt. Dar uber hinaus wurde eine Korrelation der Blutparameter mit einer h oheren Morbidit at und Mortalit at festgestellt und auf dieser Grundlage Cut-o -Werte f ur beide Blutparameter berechnet. Zus atzlich konnten weitere prognoserelevante Parameter (Geburtsgewicht, Gestationsalter, intrauterine Wachstumsretardierung, antenatale Steroidgabe, Apgar-Werte nach 5 und 10 Minuten) identi ziert und die Rolle der beiden Blutparameter im multivariaten Modell mit diesen ermittelt werden. Fazit und Diskussion In der Behandlung von sehr und extrem Fr uhgeborenen ist es hilfreich, Normwerte f ur Normoblasten- und Serumlaktat-Konzentrationen zu kennen, um diese in die Einsch atzung des neonatalen Outcomes einzubeziehen. Neu in unserer Studie ist die hoch signi kante Assoziation der Normoblasten-Konzentration mit der Serumlaktat-Konzentration. In der aktuellen Literatur gibt es nur wenige Angaben uber Normwerte beider Blutparameter f ur sehr und extrem unreife Neonaten, daf ur jedoch einige Hinweise auf eine mit hohen Werten assoziierte erh ohte neonatale Morbidit at und Mortali at, die auch in dieser Studie festgestellt werden konnte. Weitere prognoserelevante Parameter (Geburtsgewicht, Gestationsalter, intrauterine Wachstumsretardierung, antenatale Steroidgabe, Apgar-Werte) zeigten, dass die Prognose Fr uhgeborener multifaktoriell bedingt ist. Anhand unserer Daten ist ein zus atzlicher Einsatz der beiden Blutparameter im klinischen Alltag zur Beurteilung der Prognose als e ektiv einzusch atzen. Im Vergleich ist die Serumlaktat-Konzentration ein besserer Pr adiktor f ur das neonatale Outcome als die Normoblasten-Konzentration, was auch bei Betrachtung der ermittelten Cut-o -Werte sowie der Sensitivit at und Spezi t at deutlich wurde.
1.2 Summary Nucleated red blood cells and serum lactate concentrations in very low birth weight and extremely low birth weight infants in the rst 6 days of life Introduction Nucleated red blood cells are progenitor cells of the red blood cells physiologically seen in the bone marrow of healthy infants, adolescents and adults, but may also appear in the peripheral blood of the fetus and newborn in the early postnatal period - most importantly in premature infants. The appearance of nucleated red blood cells in the peripheral blood is also an indicator of hypoxia in utero. Another important biomarker for poor neonatal outcome constitutes serum lactate. It accumulates under anaerobic conditions. Several studies have demonstrated an association between elevated nucleated red blood cells as well as serum lactate and poor neonatal outcome. Study purpose The aim of our study was to de ne reference values for nucleated red blood cells and serum lactate concentrations during the rst 6 days of life in very low birth weight (<1500 grams) and extremely low birth weight (<1000 grams) infants as well as for neonates with intrauterine growth restriction, and to assess a possible correlation between nucleated red blood cells and serum lactate concentrations. Moreover we investigated whether a potential correlation between these two parameters and neonatal morbidity and mortality exists and looked for further parameters, that in uence the neonatal outcome. On top of that we analyzed the role of the nucleated red blood cells and serum lactate concentration in the overall context with these parameters. Material and Methods This retrospective study was performed at the Neonatal Intensive Care Unit at Saarland University Hospital, Homburg/Saar, Germany. 250 very low birth weight and extremely low birth weight infants born between 01/2009 and 12/2012 were enrolled in this study. We subsequently analyzed nucleated red blood cells and serum lactate concentrations in the rst 6 days of life and further important markers relevant for neonatal morbidity and mortality. Results In total, 250 patients were included in this study (123 (49.2 %) male; 127 (50.8 %) female). Birth weight ranged from 320 - 1499 grams (mean: 1048 grams 305 grams). We found a high correlation between nucleated red blood cells and serum lactate concentrations. Moreover, we could demonstrate an inverse correlation between the two parameters and birth weight as well as gestational age. Neonates with intrauterine growth restriction had signi cantly higher nucleated red blood cell and serum lactate concentrations. A set of reference values and percentiles for di erent birth weight classes and children with intrauterine growth restriction could be generated. On top of that we found a correlation between high nucleated red blood cells and serum lactate concentrations and poor neonatal outcome and determined cut-o values for both parameters. Finally we could nd out that further parameters (birth weight, gestational age, intrauterine growth restriction, antenatal steroids, Apgar-Score after 5 and 10 minutes) additionally in uence the neonatal outcome. Conclusion and Discussion Knowledge of \normal" nucleated red blood cells and serum lactate concentrations is helpful because elevated values may be associated with poor neonatal outcome in preterm infants. This is one of only a few reports that de ned systematically a set of reference values for both, nucleated red blood cells and serum lactate concentrations in very and extremely low birth weight infants for the rst 6 days of life. Moreover, we highlighted the signi cant correlation between nucleated red blood cells and serum lactate concentrations in the early postnatal period and emphasized the meaning of both parameters as important biomarkers in this susceptible cohort. On top of that, we found out that many further parameters in uence the neonatal outcome and discussed the possibility to insert the nucleated red blood cells and serum lactate concentration in the clinical daily routine in addition to well-established clinical procedures. In comparison, we could demonstrate, that the serum lactate concentration is a better predictor for poor neonatal outcome than the nucleated red blood cells concentration.
