Abstract: Zusammenfassung Vaskul are E ekte einer Herzfrequenzreduktion mit Ivabradin bei Patienten mit chronisch stabiler koronarer Herzerkrankung HINTERGRUND Epidemiologische und klinische Untersuchungen belegen eine Assoziation zwischen Herzfrequenz und kardiovaskul arer Mortalit at. Experimentelle Studien wiesen direkte vaskul are E ekte einer Herzfrequenzreduktion mit dem I(f )-Kanal-Inhibitor Ivabradin nach. Die E ekte einer Herzfrequenzreduktion auf die Endothelfunktion, vaskul are Compliance und die aortale Distensibilit at von Patienten mit koronarer Herzerkrankung wurden daher im Rahmen einer prospektiven, Placebo kontrollierten klinischen Studie untersucht. METHODEN UND ERGEBNISSE 23 Patienten (m = 18, w = 5) mit einer Ruheherzfrequenz (Hf) >70 Schl agen pro Minute (englisch: beats per minute (bpm)) und einer angiographisch nachgewiesenen koronaren Herzerkrankung wurden in die Studie eingeschlossen. Im Rahmen eines Cross-over-Designs wurden alle Patienten jeweils uber 6 Monate mit Ivabradin (2 x 7,5 mg) und Placebo behandelt. Die Behandlung f uhrte zu einer Reduktion der Hf um 11,42 bpm (Iva 58; 78 8; 21 bpm vs Placebo 70; 20 8; 26 bpm, p<0; 0001), der Blutdruck blieb unver andert. Mittels Applanationstonometrie (SphygmoCorR ) wurden Augmentationsindex (Ai(x)), Augmentationsdruck (AP) und die Pulswellengeschwindigkeit (PWV) gemessen. Die Reduktion der Hf f uhrte zu einem Anstieg des Ai(x) um 12,35% (Iva 24; 29 10; 51% vs Placebo 21; 29 10; 10%, p<0; 05) und des AP um 25,65% (Iva 17; 62 8; 50 mmHg vs Placebo 13; 10 7; 37 mmHg, p<0; 001). Unter Ivabradin zeigte sich eine Reduktion der PWV um 14,05% (Iva 6; 30 1; 71 m/s vs Placebo 7; 33 1; 42 m/s, p<0; 01). Durch Messung der endothelabh angigen Fluss-vermittelten Vasodilatation (FMD) der A. brachialis wurde die Endothelfunktion bestimmt. Die Senkung der Hf durch Ivabradin f uhrte zu einer Zunahme der FMD um 18,49% (Iva 7; 30 2; 15% vs Placebo 5; 95 2; 00%, p<0; 001). Die Bestimmung der Distensibilit at der Aorta ascendens erfolgte mittels Magnetresonanztomogra e (MRT). Hierzu wurden Cine- SSFP-Sequenzen senkrecht zur Aorta ascendens auf H ohe der rechten Pulmonalarterie (TR / TE = 58,1 / 1,3 ms, FA = 60 , FOV = 20x20 cm, Matrix = 192x192) sowie usssensitive Phasenkontrastsequenzen (TR / TE = 42,8 / 3,1 ms, FA = 20 , FOV = 20x30 cm, Matrix = 192x192) mittels retrospektivem EKG-Gating akquiriert. Die Reduktion der Herzfrequenz mit Ivabradin f uhrte zu einer Steigerung der aortalen Distensibilit at um 33,33% (Iva 0; 003 0; 001 mmHg􀀀1 vs Placebo 0; 002 0; 010 mmHg􀀀1, p<0; 01) und der zyklischen zirkumferentiellen Deformation der Aorta ascendens (circumferential cyclic strain) um 37,10% (Iva 0; 062 0; 027 vs Placebo 0; 039 0; 018, p<0; 0001). Die Behandlung mit Ivabradin f uhrte zu einer verminderten Expression des Adh asionsmolek uls ICAM-1 (Iva 292; 41 70; 41 ng/ml vs Placebo 314; 55 81; 13 ng/ml, p<0; 05): SCHLUSSFOLGERUNGDie Reduktion der Herzfrequenz mit Ivabradin f uhrt bei Patienten mit stabiler KHK zu einer Reduktion der Pulswellengeschwindigkeit und zu einer verbesserten endothelabh angigen Vasodilatation. Dar uber hinaus zeigte sich eine Steigerung der aortalen Distensibilit at. Zusammenfassend vermittelt die Reduktion der Herzfrequenz eine Verbesserung der Endothelfunktion und der vaskul aren Compliance. Die Ergebnisse unterstreichen die im Rahmen experimenteller Studien erhobenen Befunde und weisen auf die Bedeutung der Herzfrequenz im Rahmen der kardiovaskul aren Prim ar- und Sekund arpr avention hin.
Abstract Vascular e ects of heart rate reduction with Ivabradine in patients with chronic stable coronary artery disease BACKGROUND Epidemiological and clinical studies have shown a relevant association between heart rate and cardiovascular mortality. Experimental studies demonstrated direct vascular e ects of heart rate reduction with the I(f )-channel inhibitor Ivabradine. Therefore the e ects of a heart rate reduction on endothelial function, vascular compliance and aortic distensibility of patients with coronary artery disease were examined in a prospective, placebo-controlled clinical trial. METHODS AND RESULTS 23 patients (m = 18, w = 5) with a resting heart rate (HR) >70 beats per minute (bpm) and angiographically proven coronary artery disease were enrolled in this study. As part of a cross-over design, all patients were treated with Ivabradine (2 x 7,5 mg) and placebo, each for 6 months. The treatment resulted in an HR reduction of 11.42 bpm (Iva 58:78 8:21 bpm vs placebo 70:20 8:26 bpm, p<0:0001), the blood-pressure remained unchanged. Augmentation index (Ai(x)), augmentation pressure (AP) and pulse wave velocity (PWV) were measured by aplanation tonometry (SphygmoCorR ). Heart rate reduction caused an increase of Ai(x) of 12.35% (Iva 24:29 10:51% vs Placebo 21:29 10:10%, p<0:05) and of AP of 25.65% (Iva 17:62 8:50 mmHg vs placebo 13:10 7:37 mmHg, p<0:001). Ivabradine led to a reduction of PWV of 14.05% (Iva 6:30 1:71 m/s vs placebo 7:33 1:42 m/s, p<0:01). Endothelial function was determined by measuring the ow-mediated vasodilation (FMD) of the brachial artery. The reduction of HR by Ivabradine led to an increase of FMD of 18.49% (Iva 7:30 2:15% vs placebo 5:95 2:00%, p<0:001). The determination of the aortic distensibility was performed by magnetic resonance imaging (MRI). For this purpose, cine SSFP sequences were acquired perpendicularly to the ascending aorta at the level of the right pulmonary artery (TR / TE = 58.1 / 1.3 ms, FA = 60 , FOV = 20x20 cm, matrix = 192x192) and ow sensitive phase contrast sequences (TR / TE = 42.8 / 3.1 ms, FA = 20 , FOV = 20x30 cm, matrix = 192x192) were acquired by retrospective ECG gating. Heart rate reduction led to an increase of aortic distensibility of 33.33% (Iva 0:003 0:001 mmHg􀀀1 vs placebo 0:002 0:010 mmHg􀀀1, p<0:01) and of the circumferential cyclic strain of 37.10% (Iva 0:062 0:027 vs placebo 0:039 0:018, p<0:0001). Treatment with Ivabradine led to a lower expression of the adhesion molecule ICAM- 1 (Iva 292:41 70:41 ng/ml vs Placebo 314:55 81:13 ng/ml, p<0:05): CONCLUSION Heart rate reduction with Ivabradine in patients with stable coronary artery disease leads to a reduction in pulse wave velocity and improves endothelium-dependent vasodilation. In addition, an increase in aortic distensibility was shown. In summary, the reduction of the heart rate leads to an improvement of the endothelial function and the vascular compliance. The ndings con rm the results collected within the scope of experimental studies and indicate the importance of the heart rate in primary and secondary cardiovascular prevention.
